半夏厚朴汤加味治疗咽部异感症(脾虚肝郁证)的疗效及对血液流变学和炎症因子的影响

Modified Banxia Houpo Decoction Regulates Hemorheological Indexes and Inflammatory Cytokines in Patients with Abnormal Sensation of Pharynx(Syndrome of Spleen Deficiency and Liver Depression)

目的 探讨半夏厚朴汤加味治疗咽部异感症(脾虚肝郁证)的疗效及对患者血液流变学和炎症因子的作用。方法 用随机数字表法将2020年1月—2022年3月就诊于临沂市中医医院的咽部异感症(脾虚肝郁证)患者92例分为对照组(46例)和观察组(46例),对照组采用奥美拉唑肠溶片治疗,观察组采用半夏厚朴汤加味治疗,两组均治疗2个月。治疗后,观察两组治疗有效率、相关指标及不良反应发生情况。结果 观察组治疗有效率高于对照组(P <0.05);治疗后,观察组的咽部异物感、中医证候评分,血液流变学指标(全血低切黏度、纤维蛋白原、血浆黏度、全血高切黏度、红细胞压积)及炎症因子中的白细胞介素-6、肿瘤坏死因子-α、C反应蛋白水平均低于对照组(P <0.01),生活质量评分及炎症因子中的白细胞介素-2水平均高于对照组(P <0.01);观察组不良反应发生率低于对照组(P <0.05)。结论 咽部异感症(脾虚肝郁证)应用半夏厚朴汤加味治疗,可提高治疗有效率,调节患者的血液流变学指标,降低炎症反应。

Objective To investigate the efficacy of modified Banxia Houpo Decoction in treatingthe patients with abnormal sensation of pharynx(syndrome of spleen deficiency and liver depression)and the effects of the decoction on hemorheological indexes and inflammatory cytokines.Methods A total of 92 patients with abnormal sensation of pharynx treated in the Linyi Traditional Chinese Medical Hospital from January 2020 to March 2022 were assigned into a control group(n=46)and an observation group(n=46)by the random number table method.The control group was treated with Omeprazole Enteric-coated Tablets and the observation group with Modified Banxia Houpo Decoction.Each course of treatment lasted for 2 months.The response rate,related indexes,and incidence of adverse reactions were compared between the two groups.Results The response rate of the observation group was higher than that of the control group(P<0.05).After treatment,the abnormal sensation score of pharynx,TCM syndrome score,hemorheological indexes(whole blood viscosity at low shear,fibrinogen,plasma viscosity,whole blood viscosity at high shear,and hematocrit),and levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),and C-reactive protein(CRP)in the observation group were lower than those in the control group(P<0.01).The quality of life score and the level of interleukin-2(IL-2)in the observation group were higher than those in the control group(P<0.01).In addition,the observation group had lower incidence of adverse reactions than the control group(P<0.05).Conclusion Modified Banxia Houpo Decoction can significantly improve the response rate,regulate the hemorheological indexes,and reduce inflammation in the patients with abnormal sensation of pharynx(syndrome of spleen deficiency and liver depression).
