中老年人巨噬细胞迁移抑制因子和白细胞介素-6与牙周炎及动脉粥样硬化相关性的研究

A preliminary study on the relationship between macrophage migration inhibitory factor,interleukin-6 and Periodontal disease,atherosclerosis in middle aged and old people

目的探讨中老年人巨噬细胞迁移抑制因子(MIF)、白细胞介素-6(IL-6)与牙周炎、动脉粥样硬化的相关性。方法横断面研究,2020年1月至2022年12月入选北京医院内分泌科就诊的糖尿病患者700例,其中491例接受全口牙周检查。106例为中老年人,83例资料完整患者纳入本研究,年龄45~70(58.2±8.7)岁,其中男性48例(57.8%),女性35例(42.2%)。问卷调查了解患者一般情况,酶联免疫吸附试验(ELISA)检测血清中MIF、IL-6水平。单因素分析和多因素分析评估中老年人中血清MIF、IL-6含量与牙周炎、动脉粥样硬化的相关性。结果牙周炎分期Ⅰ/Ⅱ期组66例(79.5%),Ⅲ/Ⅳ期组17例(20.5%),两组血清MIF水平为(2312.5±795.0)ng/L比(2939.8±665.0)ng/L(P<0.01),IL-6为(3.5±3.0)ng/L比(6.7±6.1)ng/L(P>0.05)。MIF血清含量每增加1 ng/L,患重度牙周炎的风险和颈动脉内膜中层厚度(cIMT)增加风险均增加0.1%;血清MIF、IL-6含量每增加1 ng/L,颈动脉斑块风险分别增加0.2%和38.9%。血清MIF水平与动脉粥样硬化相关,与牙周炎分期、牙周探诊深度、出血指数、临床附着水平、牙周探诊深度≥4 mm位点数、临床附着水平≥5%、出血指数>2%亦相关(均P<0.05);血清IL-6水平,与牙周炎分期、临床附着水平、出血指数>2%相关(均P<0.05)。结论血清MIF、IL-6水平与中老年人牙周炎、动脉粥样硬化相关。

Objective To examine the association between macrophage migration inhibitory factor(MIF),Interleukin 6(IL-6),periodontal disease,and atherosclerosis in middle-aged and elderly individuals.Methods This cross-sectional study enrolled 700 diabetic patients who were attending the endocrinology department of Beijing Hospital from January 2020 to December 2022.Out of these patients,491 underwent a full-mouth periodontal examination.Among them,106 were middle-aged and elderly.The study included 83 patients with complete data,aged between 45 and 70 years(mean age:58.2±8.7 years).Among the included patients,48 were male(57.8%)and 35 were female(42.2%).A questionnaire was administered to gather information about the patients,and serum levels of MIF and IL-6 were measured using enzyme-linked immunosorbent assay(ELISA).The relationship between serum MIF and IL-6 levels and periodontitis and atherosclerosis was evaluated through univariate and multifactorial analyses conducted on middle-aged and elderly individuals.Results There were 66 cases(79.5%)in the periodontitis stageⅠ/Ⅱgroup and 17 cases(20.5%)in the stageⅢ/Ⅳgroup.The serum MIF levels in the two groups were(2,312.5±795.0)ng/L and(2,939.8±665.0)ng/L,respectively(P<0.01).Similarly,IL-6 levels were(3.5±3.0)ng/L and(6.7±6.1)ng/L in the two groups(P>0.05).The findings revealed that for each 1 ng/L increase in MIF serum levels,the risk of severe periodontitis and increased carotid intima-media thickness(cIMT)increased by 0.1%.Furthermore,for each 1 ng/L increase in serum MIF and IL-6 levels,the risk increased by 0.2%and 38.9%respectively.Serum MIF levels were found to be associated with atherosclerosis as well as with various factors related to periodontitis,including periodontal probing depth,bleeding index,clinical attachment level,periodontal probing depth≥4 mm sites,clinical attachment level≥5%,and bleeding index>2%(all P<0.05).Conclusions This study investigates the correlation between MIF and IL-6 levels with Periodontal disease and Atherosclerosis in middle-aged and elderly individuals.
