关节镜下自体腓骨长肌腱重建对前交叉韧带损伤患者运动功能及血清骨桥蛋白(OPN)、CD44水平的影响

摘要 目的探究并分析关节镜下自体腓骨长肌腱重建对前交叉韧带损伤患者运动功能及血清骨桥蛋白(O PN)、CD44水平的影响。方法选取2020年2月—2021年2月在我院治疗的57例合并内侧副韧带损伤的前交叉韧带损伤患者,按照随机数字表法将其分组(观察组28例及对照组29例)。对照组采用腘绳肌腱(股薄肌腱以及半腱肌腱)进行交叉韧带重建,观察组采用自体腓骨长肌腱重建。观察并记录二组患者一般资料,比较二组患者治疗前、治疗后1个月,6个月Lysholm评分,Kofoed评分,血清骨桥蛋白(osteopontin,OPN)水平,血清CD44水平。结果治疗后6个月观察组患者Kofoed评分明显高于对照组(98.47±5.35vs.88.42±5.27,P<0.05),治疗后6个月观察组患者Lysholm评分明显高于对照组(88.47±5.35vs.73.42±5.27,P<0.05),治疗后6个月观察组患者OPN水平明显低于对照组(8.43±4.41vs.15.37±4.36,P<0.05),治疗后6个月观察组患者CD44水平明显低于对照组(168.75±35.41vs.245.37±32.36,P<0.05),具有统计学意义。结论关节镜下自体腓骨长肌腱重建能有效改善前交叉韧带损伤患者运动功能,降低血清骨桥蛋白(OPN)、CD44水平,值得临床推广。 Objective To investigate and analyze the effects of arthroscopic reconstruction of the peroneal tendon on motor function and serum osteopontin(OPN) and CD44 levels in patients with anterior cruciate ligament injury. Methods Fifty-seven patients with anterior cruciate ligament injury combined with medial collateral ligament injury who were treated in our hospital from February 2020 to February 2021 were randomly divided into two groups(observation group of twenty-eight cases and control group of twenty-nine cases).The control group used hamstring tendons(gracilis tendon and semitendinosus tendon) for cruciate ligament reconstruction, while the observation group used autologous peroneal longus tendon reconstruction.Observe and record the general information of the two groups of patients, and compare the Lysholm score, Kofoed score, serum osteopontin(OPN) level, and serum CD44 level of the two groups of patients before and after treatment. Results After treatment, the Kofoed score of patients in the observation group was significantly higher than that in the control group(98.47 ± 5.35 vs. 88.42 ± 5.27,P<0.05),and the Lysholm score of patients in the observation group was significantly higher than that in the control group(88.47 ± 5.35 vs. 73.42 ± 5.27,P<0.05).After treatment, the OPN level of patients in the observation group was significantly lower than that in the control group(8.43 ± 4.41 vs. 15.37 ± 4.36,P<0.05),After treatment, the CD44 level in the observation group was significantly lower than that in the control group(168.75 ± 35.41 vs. 245.37 ± 32.36,P<0.05),with statistical significance. Conclusion Arthroscopic reconstruction of autologous peroneal tendon can effectively improve motor function and reduce serum osteopontin(OPN) and CD44 levels in patients with anterior cruciate ligament injury, which is worthy of clinical promotion.
