重型颅脑损伤气管切开患者脱机序贯高流量湿化氧疗的效果研究

Effect of offline sequential high flow humidified oxygen therapy on patients with severe craniocerebral injury undergoing tracheotomy

摘要 目的 研究重型颅脑损伤气管切开患者脱机序贯高流量湿化氧疗的临床效果。方法选取该院2020年1月—2021年12月收治的120例重型颅脑损伤气管切开患者作为研究对象,观察组(n=60)给予高流量湿化氧疗,对照组(n=60)给予常规湿化氧疗。比较二组患者住ICU期间肺部感染发生率、肺部感染控制时长、气道并发症发生率。结果 入院时二组患者基本资料、GCS评分均无明显统计学差异(P>0.05);观察组的肺部感染发生率为8.33%显著低于对照组30.00%,差异有统计学意义(P<0.05);观察组的肺部感染控制时长(7.00±0.71)d显著优于对照组患者的控制时长(9.11±2.00)d,差异有统计学意义(P<0.05);观察组气道并发症发生率(痰液阻塞发生率、气道黏膜损伤发生率)与对照组相比显著降低,差异有统计学意义(P<0.05)。结论 重型颅脑损伤气管切开患者脱机序贯高流量湿化氧疗与常规湿化氧疗对比,能降低肺部感染发生率、缩短肺部感染控制时长和减少气道并发症发生。 Objective To study the clinical effect of offline sequential high flow humidified oxygen therapy in patients with severe craniocerebral injury undergoing tracheotomy. Methods 120 patients with severe craniocerebral injury undergoing tracheotomy were selected as the test subjects.The study group(n=60) was given high flow humidified oxygen therapy, and the control group(n=60) was given conventional humidified oxygen therapy.The incidence of pulmonary infection, the duration of pulmonary infection control, and the incidence of airway complications were compared between the two groups. Results There was no significant difference in basic data and GCS scores between the two groups at admission(P>0.05).The incidence of pulmonary infection in the study group was 8.33%,the duration of pulmonary infection control was(7.00±0.71) days, and the incidence of airway complications(the incidence of sputum obstruction and the incidence of airway mucosal injury).There were significant differences between the study group and the control group(P<0.05). Conclusion Compared with routine humidified oxygen therapy, off-line sequential high flow humidified oxygen therapy can reduce the incidence of pulmonary infection, shorten the control time of pulmonary infection and reduce the occurrence of airway complications in patients with severe craniocerebral injury undergoing tracheotomy.
