护士主导的结构化教育课程在慢性非癌性疼痛患者中的应用效果

摘要 目的探讨护士主导的结构化教育课程在慢性非癌性疼痛患者疼痛管理中的应用效果。方法选取2021年1—6月医院收治的慢性非癌性疼痛患者100例为研究对象,按基本资料组间匹配原则将患者分为对照组和观察组,各50例。对照组采用常规护理措施进行干预,观察组在对照组基础上予以护士主导的结构化教育课程。以视觉模拟量表(VAS)评价患者入院时、出院时及出院1个月时的疼痛程度。采用自制问卷,在出院时对患者疼痛及镇痛知识进行评估。结果护理干预前,两组患者VAS评分、患者的自我效能、患者相关知识掌握情况比较,差异无统计学意义(P>0.05)。护理干预后,观察组患者的VAS评分低于对照组,观察组患者的自我效能在疼痛管理自我效能、躯体性自我效能以及症状应对自我效能方面都优于对照组,差异有统计学意义(P<0.05)。观察组患者在对疼痛的理解、镇痛药物相关知识的掌握程度优于对照组,差异有统计学意义(P<0.05)。结论护士主导的结构化教育课程能够增加慢性非癌性疼痛患者对疼痛认知和对镇痛药物的认识,提升患者的自我效能,减轻患者短期及长期疼痛。 Objective To explore the application effect of nurse-led structured education programme in pain management of patients with chronic non-cancer pain.Methods 100 cases of chronic non-cancer pain patients admitted to the hospital from January to June 2021 were selected as the study subjects,they were divided into a control group and an observation group of 50 cases each according to the principle of matching basic data between groups.The control group was intervened with conventional nursing measures,and the observation group was given a nurse-led structured education programme on the basis of the control group.Visual Analogue Scale(VAS)was used to evaluate the patients'pain level at admission,discharge and 1 month after discharge.A self-administered questionnaire was used to assess patients'pain and analgesic knowledge at discharge.Results Before the nursing intervention,there was no statistically significant diff erence between the two groups in terms of VAS scores,patients'self-efficacy,and patients'related knowledge(P>0.05).After nursing intervention,the VAS scores of patients in the observation group were lower than those of the control group,and the self-efficacy of patients in the observation group was better than that of the control group in terms of pain management self-efficacy,somatic self-efficacy,and symptom coping self-efficacy,and the diff erence was statistically significant(P<0.05).The patients in the observation group were better than the control group in terms of their understanding of pain and knowledge related to analgesic drugs,the diff erence was statistically significant(P<0.05).Conclusion Nurse-led structured education programmes can increase chronic non-cancer pain patients'understanding of pain and knowledge of analgesic medications,improve patients'self-efficacy,and reduce short and long-term pain.
