摘要
目的:探讨剪切波弹性成像(SWE)联合二维超声引导穿刺对乳腺癌诊断的临床价值。方法:选取医院收治的400例疑似乳腺癌患者,对所有患者乳腺的疑似恶性肿瘤部位均行普通二维超声引导穿刺及SWE联合二维超声引导穿刺活检,以病理检查结果作为判断良恶性的“金标准”,分析SWE联合超声引导穿刺对乳腺癌诊断的临床价值。结果:病理结果显示,400例疑似乳腺癌患者中确诊恶性164例,良性236例。二维超声引导穿刺诊断阳性128例;SWE联合二维超声引导穿刺诊断阳性160例;采取SWE联合二维超声引导穿刺活检对乳腺癌诊断的灵敏度和准确率分别为94%和96%,均高于单一超声引导穿刺活检,差异有统计学意义(x^(2)=12.401,x^(2)=10.960;P<0.05)。结论:SWE联合二维超声引导穿刺活检对乳腺癌的诊断价值突出,可有效鉴别乳腺良恶性肿瘤。
Objective:To explore the clinical value of shear wave elastography(SWE)combined with two-dimensional(2D)ultrasound-guided puncture in the diagnosis of breast cancer.Methods:A total of 400 patients with suspected breast cancer who admitted to hospital were selected,and the sites of suspected malignant tumor of the breast of all patients underwent puncture biopsy by both ordinary 2D ultrasound-guided puncture and puncture biopsy with SWE combined with ordinary 2D ultrasound-guided.The result of pathological examination was used as the"Gold standard"of judging benign and malignant tumors.The clinical value of SWE combined with ultrasound-guided puncture in the diagnosis of breast cancer was analyzed.Results:The results of pathological examination indicated that 164 cases were confirmed as malignant tumor and 236 cases were confirmed as benign tumor in 400 patients with suspended breast cancer.A total of 128 cases were diagnosed as positivity by 2D ultrasound-guided puncture,and 160 cases were diagnosed as positivity by SWE combined with 2D ultrasound-guided puncture.The sensitivity and accuracy of SWE combined with ordinary 2D ultrasound-guided puncture biopsy were significantly higher than those of single ultrasound puncture biopsy in diagnosing breast cancer(x^(2)=12.401,x^(2)=10.960,P<0.05),and the differences of them were significant,respectively.Conclusion:SWE combined with ordinary 2D ultrasound-guided puncture biopsy has outstanding diagnostic value for breast cancer,which can effectively distinguish benign and malignant breast tumors.
