摘要
目的:探讨基于全过程协同管理的医疗设备全生命周期精细化管理在医疗设备质量评估与质量-控制中的应用价值。方法:从医疗设备的选型论证、采购验收、维修保养和报废处置4个阶段实施医疗设备全生命周期精细化管理。选取医院临床在用的104台医疗设备,按照管理方法不同分为观察组和对照组,观察组实施全生命周期精细化管理,对照组实施常规维护。自制调查量表对设备操作和维修人员开展调查,对比分析设备故障情况、4个阶段的维度得分(选型论证、采购验收、维修保养和报废处置)、非设备影响因素和影响设备故障事件种类数量因素。结果:观察组定期保养不及时和设备故障种类数量发生率分别为1.92%(2/104)和9.62%(10/104),均低于对照组,差异有统计学意义(x^(2)=8.694,x^(2)=24.787;P<0.05)。观察组设备的选型论证、采购验收、维修保养和报废处置得分分别为(12.71±2.19)分、(15.87±2.69)分、(20.23±3.06)分和(15.61±3.00)分,均高于对照组,差异有统计学意义(t=24.321,t=17.447,t=22.254,t=15.88;P<0.05)。观察组有3台设备维修与使用人员发生工作变动,有101台设备健全应急预案设定,人员变动低于对照组,应急预案设定高于对照组,差异有统计学意义(x^(2)=4.021,x^(2)=4.901;P<0.05)。影响设备故障事件种类数的因素有设备维修与使用人员发生工作变动、健全应急预案设定、选型论证、维修保养和报废处置。结论:医疗设备全生命周期精细化管理能够降低设备故障发生率,减少单一设备故障事件种类的发生。
Objective:To discuss the application value of medical equipment lifecycle refinement management based on whole process collaborative management in medical equipment quality assessment and control.Methods:The refined management of the whole life cycle of medical equipment was implemented from the four stages of medical equipment selection and demonstration,procurement acceptance,maintenance and scrap disposal.A total of 104 sets of medical equipment in clinical use in the hospital were selected and divided into observation group and control group according to different management methods.The observation group implemented refined management of the whole life cycle,and the control group implemented routine maintenance.The self-made survey scale was used to investigate equipment operation and maintenance personnel,the equipment failure conditions,stage dimension scores,nonequipment influencing factors and factors affecting the number of types of equipment failure events were compared and analyzed.Results:The incidence rates of untimely regular maintenance and the number of types of equipment failures in the observation group were 1.92%(2/104)and 9.62%(10/104),respectively,which were lower than those in the control group,and the difference was statistically significant(x^(2)=8.694,x^(2)=24.787;P<0.05).The scores of medical equipment model selection demonstration,purchase acceptance,maintenance and scrap disposal of the observation group were(12.71±2.19)points,(15.87±2.69)points,(20.23±3.06)points and(15.61±3.00)points,which were higher than those of the control group,the difference was statistically significant(t=24.321,t=17.447,t=22.254,t=15.88;P<0.05).In the observation group,there were 3 equipment maintenance and operation personnel changed,101 equipment had completed emergency plan setting,the personnel change was lower than that in the control group,the emergency plan setting was higher than that in the control group,the difference was statistically significant(x^(2)=4.021,x^(2)=4.901;P<0.05).Factors affecting the number of types of equipment failure incidents included personnel change in the maintenance and use of equipment,improvement of emergency preplan setting,model selection and demonstration,maintenance and repair as well as disposal of discarding.Conclusion:The refined management of the whole life cycle of medical equipment can reduce the incidence of equipment failure and reduce the occurrence of single equipment failure event types.
