噻托溴铵联合布地奈德福莫特罗治疗慢性阻塞性肺疾病的效果及对血气分析指标、miR-145、miR-146a的影响

摘要 目的 探讨噻托溴铵联合布地奈德福莫特罗治疗慢性阻塞性肺疾病(COPD)的效果。方法 选择2021年3月至2022年1月收治的86例COPD患者作为研究对象,按照随机数字表法将其分为对照组与观察组,各43例。对照组给予布地奈德福莫特罗治疗,观察组在对照组基础上给予噻托溴铵治疗。比较两组的治疗效果。结果 观察组的治疗总有效率明显高于对照组(P<0.05)。治疗后,观察组的动脉血氧分压(PaO_(2))、血氧饱和度(SaO_(2))高于对照组,动脉血二氧化碳分压(PaCO_(2))低于对照组(P<0.05)。治疗后,观察组的英国医学研究委员会呼吸困难量表(mMRC)、COPD评估测试(CAT)评分均低于对照组(P<0.05)。治疗后,观察组的miR-145、miR-146a表达水平均高于对照组(P<0.05)。结论 噻托溴铵联合布地奈德福莫特罗治疗COPD的效果满意,可改善患者的血气分析指标与临床症状,调节miR-145、miR-146a表达。 Objective To investigate the effect of tiotropium bromide combined with budesonide formoterol in the treatment of chronic obstructive pulmonary disease(COPD).Methods A total of 86 patients with COPD admitted from March 2021 to January 2022 were selected as the research objects and divided into control group and observation group according to the random number table method,with 43 cases in each group.The control group was treated with budesonide formoterol,and the observation group was treated with tiotropium bromide on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was significantly higher than that in the control group(P<0.05).After treatment,the arterial partial pressure of oxygen(PaO_(2))and blood oxygen saturation(SaO_(2))in the observation group were higher than those in the control group,and the arterial partial pressure of carbon dioxide(PaCO_(2))was lower than that in the control group(P<0.05).After treatment,the scores of Modified Medical Research Council Scale(mMRC)and COPD Assessment Test(CAT)in the observation group were lower than those in the control group(P<0.05).After treatment,the expression levels of miR-145 and miR-146a in the observation group were higher than those in the control group(P<0.05).Conclusion The tiotropium bromide combined with budesonide formoterol in the treatment of COPD has a satisfactory effect.It can improve the blood gas analysis indexes and clinical symptoms of patients,and regulate the expression of miR-145 and miR-146a.
