不同类型肺癌患者双层探测器光谱CT检查定量参数对比分析

Comparative analysis of quantitative parameters in dual detector spectral CT examination of different types of lung cancer patients

目的:分析对比不同类型肺癌患者的双层探测器光谱CT参数差异。方法:回顾性分析2021年9月—2022年9月山东国欣颐养集团枣庄中心医院收治的84例肺癌患者的临床资料,根据病理检查结果将其分为非小细胞肺癌(NSCLC)组(n=63)与小细胞肺癌(SCLC)组(n=21)。所有患者均行双层探测器光谱CT检查,统计对比两组动脉期、静脉期的各项双层探测器光谱CT参数差异。结果:NSCLC组动脉期、静脉期的有效原子序数(Zeff)、碘浓度(IC)、标准化碘浓度(NIC)、光谱曲线斜率(K_(40-100keV))均高于SCLC组(P<0.05);NSCLC组病灶形态为分叶/毛刺、支气管狭窄闭塞占比高于SCLC组,病灶形态为锥状形/纺锤形、与支气管长轴平行占比低于SCLC组(P<0.05);两组病灶形态不规则形、均匀强化占比相当,差异无统计学意义(P>0.05)。结论:NSCLC与SCLC的双层探测器光谱CT各项检查参数存在一定差异,且两者的影像学表现亦不相同,双层探测器光谱CT能够为临床筛查、鉴别不同类型肺癌提供一定的参考信息。

Objective To analyze and compare the differences in spectral CT parameters of dual detector in different types of lung cancer patients.Methods A retrospective analysis was conducted on the clinical data of 84 lung cancer patients admitted to Zaozhuang Central Hospital of Shandong Guoxin Yiyang Group from September 2021 to September 2022.Based on pathological examination results,they were divided into non-small cell lung cancer(NSCLC)group(n=63)and small cell lung cancer(SCLC)group(n=21).All patients underwent double-layer detector spectral CT examination,and the differences in various double-layer detector spectral CT parameters between the two groups during the arterial and venous phases were statistically compared.Results In NSCLC group,the effective atomic number(Zeff),iodine concentration(IC),normalized iodine concentration(NIC)and spectral curve slope(K_(40-100keV))in arterial phase and venous phase were higher than those in SCLC group(P<0.05);In NSCLC group,the proportion of lesions with lobulated/spiculated shape and bronchial stenosis and occlusion was higher than that in SCLC group,while the proportion of lesions with conical/Fusiform shape and parallel to the long axis of bronchus was lower than that in SCLC group(P<0.05);The proportion of irregularly shaped and uniformly enhanced lesions in the two groups was similar,and the difference was not statistically significant(P>0.05).Conclusion There are certain differences in the examination parameters of NSCLC and SCLC's dual detector spectral CT,and their imaging manifestations are also different.Dual detector spectral CT can provide certain reference information for clinical screening and differentiation of different types of lung cancer.
