经腹部与经阴道超声联合诊断异位妊娠的临床应用价值

Clinical application of transabdominal and transvaginal ultrasonography in diagnosis of ectopic pregnancy

目的:探讨经腹部与经阴道超声联合诊断异位妊娠的临床应用效果。方法:选取2020年3月—2022年12月青岛市即墨区第二人民医院收治的异位妊娠患者116例,均经临床病理检查确诊,所有患者均接受经腹部与经阴道超声诊断。比较经腹部与经阴道超声联合及单一诊断的结果,比较不同方式检测不同类型异位妊娠的检出情况及声像图情况。结果:经腹部与经阴道超声联合检出率82.76%均明显高于经腹部超声的70.69%及经阴道超声的75.86%,差异有统计学意义(P<0.05);经腹部与经阴道超声联合诊断不同类型异位妊娠符合率明显高于经腹部超声经阴道超声,差异有统计学意义(P<0.05);经腹部与经阴道超声联合诊断附件包块、宫内假孕囊、盆腔积液、心管博动、胎芽及胎心、卵黄囊检出率均明显高于单一诊断,差异有统计学意义(P<0.05)。结论:经腹部与经阴道超声联合诊断异位妊娠的临床应用效果较好,可以提高临床诊断准确性,降低误诊率,值得推广应用。

Objective To investigate the clinical effect of combined transabdominal and transvaginal ultrasonography in the diagnosis of ectopic pregnancy.Methods A total of 116 patients with ectopic pregnancy admitted to the Second People's Hospital of Jimo District of Qingdao from March 2020 to December 2022 were selected,all of whom were confirmed by clinicopathological examination,and all of whom received transabdominal and transvaginal ultrasound diagnosis.The results of transabdominal and transvaginal ultrasonography combined with single diagnosis were compared,and the detection and ultrasound images of different types of ectopic pregnancy detected by different methods were compared.Results The detection rate of transabdominal and transvaginal ultrasonography(82.76%)was significantly higher than that of transabdominal ultrasonography(70.69%)and transvaginal ultrasonography(75.86%),and the difference was statistically significant(P<0.05).The coincidence rate of transabdominal and transvaginal ultrasonography for different types of ectopic pregnancy was 82.76%,which was significantly higher than that of transabdominal ultrasonography(70.69%)and transvaginal ultrasonography(75.86%),and the difference was statistically significant(P<0.05).The detection rates of adnexal mass,intrauterine pregnancy sac,pelvic effusion,cardiac tube movement,fetal buds,fetal heart and yolk sac were significantly higher than those of single diagnosis,and the difference was statistically significant(P<0.05).Conclusion Combined transabdominal and transvaginal ultrasonography can improve the accuracy of clinical diagnosis and reduce the misdiagnosis rate.It is worth applying.
