拔管时机对肺癌患者全身麻醉苏醒期复苏质量和并发症的影响

The Effect of Extubation Timing on the Quality of Resuscitation and Complications in Lung Cancer Patients During General Anesthesia Recovery Period

目的探讨拔管时机对肺癌患者全身麻醉苏醒期复苏质量和并发症的影响。方法选取2020年3月至2022年3月医院收治的110例择期行全身麻醉胸腔镜下肺癌根治术患者作为研究对象。将2020年3月至2021年3月实施完全清醒后拔管的55例患者作为对照组,将2021年4月至2022年3月实施一定麻醉深度下早期拔管的55例患者作为试验组。比较两组全身麻醉苏醒期复苏质量和并发症发生率。结果两组脱机时、拔管后10 min的平均动脉压(MAP)、心率(HR)比较,差异无统计学意义(P>0.05);试验组拔管前5 min、拔管时、拔管后5 min的MAP、HR均低于对照组,差异有统计学意义(P<0.05)。出麻醉复苏室时,两组的动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))、氧合指数比较,差异无统计学意义(P>0.05)。试验组拔管时间和麻醉复苏室滞留时间均短于对照组,拔管后并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论肺癌术后早期拔管可帮助患者平稳度过麻醉苏醒期,提高复苏质量,降低拔管后并发症发生率。

Objective To explore the effect of extubation timing on the resuscitation quality and complications of general anesthesia in lung cancer patients during the recovery period.Methods With selection of 110 patients undergoing elective general anesthesia for thoracoscopy lung cancer radical surgery admitted to the hospital from March 2020 to March 2022,among them with 55 patients who underwent extubation after complete awakening from March 2020 to March 2021 as the control group,and 55 patients who underwent early extubation at a certain depth of anesthesia from April 2021 to March 2022 as the experimental group,the quality of resuscitation and incidence of complications during the recovery period of general anesthesia were compared between two groups.Results There were no significant differences in mean arterial pressure(MAP)and heart rate(HR)between the two groups at weaning and 10 minutes after extubation(P>0.05);The MAP and HR of the experimental group were lower than those of the control group at 5 minutes before,during,and 5 minutes after extubation,with statistical significances(P<0.05).When leaving the anesthesia resuscitation room,there were no statistically significant differences in arterial oxygen partial pressure(PaO_(2)),arterial carbon dioxide partial pressure(PaCO_(2)),and oxygenation index between the two groups(P>0.05).The extubation time and anesthesia resuscitation room retention time of the experimental group were shorter than those of the control group,and the incidence of complications after extubation was lower than that of the control group,with statistical significances(P<0.05).Conclusion Early extubation after lung cancer surgery can help patients smoothly pass the anesthesia recovery period,improve the quality of resuscitation,and reduce the incidence of complications after extubation.
