乳腺彩超BI-RADS分类在乳腺结节中的临床应用 被引量:2

摘要 目的探讨彩色超声乳腺影像报告及数据系统(BI-RADS)分类在乳腺结节良恶性诊断中的应用价值。方法选择2020年9月至2022年12月医院收治的60例乳腺结节患者(共78个结节)作为研究对象。所有患者均接受病理检查及彩色超声BI-RADS分类,以病理检查结果为金标准,分析彩色超声BI-RADS分类对乳腺结节良恶性的诊断效能及与病理检查结果的一致性。结果病理检查结果显示,78个结节中良性52个,恶性26个。彩色超声BI-RADS分类结果显示,78个结节中1类28个,2类13个,3类12个,4类16个,5类9个。以病理检查结果为金标准,彩色超声BI-RADS分类的诊断灵敏度为80.77%,特异度为92.31%,准确度为88.46%,阳性预测值为90.57%,阴性预测值为84.00%。Kappa检验结果显示,彩色超声BI-RADS分类对乳腺结节良恶性的诊断结果与病理检查结果的一致性理想(Kappa=0.738,P<0.05)。结论彩色超声BI-RADS分类诊断乳腺结节良恶性的灵敏度、特异度、准确度、阳性预测值、阴性预测值均较高,且诊断结果与病理检查结果的一致性理想。 Objective The application value of color ultrasound breast imaging reporting and data system(BI-RADS)grading in the diagnosis of benign and malignant breast nodules was explored.Methods With selection of sixty patients with breast nodules(a total of 78 nodules)admitted to the hospital from September 2020 to December 2022 as the research subjects,all patients underwent pathological examination and color ultrasound BI-RADS grading.With pathological examination results as the gold standard,the diagnostic efficacy of color ultrasound BI-RADS grading for benign and malignant breast nodules and its consistency with pathological examination results were analyzed.Results Pathological examination showed that 52 of the 78 nodules were benign and 26 were malignant.The results of color ultrasound BI-RADS classification showed that 28 nodules were in grade 1,13 in grade 2,12 in grade 3,16 in grade 4,and 9 in grade 5.Based on the pathological examination results as the gold standard,the diagnostic sensitivity of color ultrasound BI-RADS grading was 80.77%,specificity was 92.31%,accuracy was 88.46%,positive predictive value was 90.57%,and negative predictive value was 84.00%.The Kappa test results showed that the consistency between the diagnostic results of color ultrasound BI-RADS grading for benign and malignant breast nodules and the pathological examination results was ideal(Kappa=0.738,P<0.05).Conclusion The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of color ultrasound BI-RADS grading in the diagnosis of benign and malignant breast nodules are high,and the consistency between the diagnostic results and pathological examination results is ideal.
