噻托溴铵治疗COPD合并Ⅱ型呼吸衰竭对患者肺功能及血气分析指标的影响

Effects of Tiotropium Bromide on Pulmonary Function and Blood Gas Indexes in Patients with COPD Combined with Respiratory Failure TypeⅡ

目的探究噻托溴铵治疗慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭对患者肺功能及血气分析指标的影响。方法选取南京市红十字医院收治的60例COPD合并Ⅱ型呼吸衰竭患者为研究对象,时间为2019年1月至2022年1月,将其划分为各30例的对照组(常规治疗及无创呼吸机辅助通气治疗)和观察组(以对照组为基础,使用噻托溴铵治疗),分组依据:随机数字表法。两组总治疗时间均为1个月。分析两组各项指标。结果相较于对照组指标(临床总有效率),观察组更高;相较于治疗前的各项指标(FVC、FEV_(1)、VC水平、FEV_(1)/FVC值及PaO_(2)、OI、SaO_(2)水平),观察组、对照组患者治疗后均呈较高趋势,且相较于对照组,观察组均处于更高水平;相较于治疗前的各项指标(TLC、PaCO_(2)水平、红细胞沉降率、WBC、血清CRP、PCT水平),观察组、对照组患者治疗后均处于较低水平,且相较于对照组,观察组均处于更低水平(P<0.05)。结论COPD合并Ⅱ型呼吸衰竭患者采用噻托溴铵治疗,对血气水平、免疫功能及肺功能的改善效果佳,可减轻炎性损伤,临床治疗效果显著,安全性较高。

Objective To investigate the effects of tiotropium bromide on lung function and blood gas indexes in patients with chronic obstructive pulmonary disease(COPD)combined with respiratory failure typeⅡ.Methods A total of 60 patients with COPD and type II respiratory failure admitted to Nanjing Red Cross Hospital were selected from January 2019 to January 2022,and they were divided into control group(conventional treatment and noninvasive ventilator-assisted ventilation treatment)and observation group(tiotropium bromide treatment based on the control group)with 30 cases in each group.The grouping basis was random number table method.The total treatment time of the two groups was one month.Analyze the indicators of the two groups.Results Compared with the control group(clinical total effective rate),the observation group is higher;Compared with the indexes before treatment(FVC,FEV_(1),VC levels,FEV_(1)/FVC values and PaO_(2),OI,SaO_(2)levels),the patients in the observation group and the control group showed a higher trend after treatment,and compared with the control group,the patients in the observation group were at a higher level.Compared with the indexes before treatment(TLC,PaCO_(2)level,erythrocyte sedimentation rate,WBC,serum CRP,PCT level),the patients in the observation group and the control group were at a lower level after treatment,and the observation group was at a lower level than the control group(P<0.05).Conclusions Treatment with tiotropium bromide in COPD patients complicated with type II respiratory failure has a good effect on improving blood gas level,immune function and lung function,and can reduce inflammatory injury.The clinical treatment effect is remarkable and the safety is high.
