基于四项行动法延伸手术室护理结合保温干预对子宫切除术患者疼痛程度及预后的影响

Effect of Extended Operating Room Nursing Combined with Insulation Intervention on Pain Degree and Prognosis of Patients Undergoing Hysterectomy Based on Four Actions

目的探讨基于四项行动法延伸手术室护理结合保温干预对行腹腔镜阴式子宫切除术患者疼痛程度及预后的影响。方法选取厦门大学附属第一医院杏林分院2021年1月至2022年12月期间收治的子宫切除术患者60例,根据患者意愿分组,对照组行常规护理,观察组接受基于四项行动法延伸手术室护理结合保温干预,比较两组疼痛度[视觉模拟评分(VAS)]、术后自主呼吸恢复时间、苏醒时间、复苏室停留时间与不良反应发生情况。结果观察组术后1 h、4 h、12 h的VAS评分低于对照组(P<0.05),而术后24 h时两组间VAS评分差异无统计学意义(P>0.05);术后观察组自主呼吸恢复时间、苏醒时间及复苏室停留时间均明显短于对照组(P<0.05);观察组术后不良反应发生率为3.33%,低于对照组的23.33%(P<0.05);观察组患者对护理满意度为100.0%,明显高于对照组的83.33%(P<0.05)。结论基于四项行动法延伸手术室护理结合保温干预可减轻子宫切除术患者术后疼痛度,加快其术后恢复速度,提升手术安全性及整体护理服务质量。

Objective To investigate the effect of extended operating room nursing combined with thermal insulation intervention based on four actions on pain degree and prognosis of patients undergoing laparoscopic vaginal hysterectomy.Methods A total of 60 patients with hysterectomy admitted to Xinglin Branch of the First Affiliated Hospital of Xiamen University from January 2021 to December 2022 were grouped according to their wishes.The control group received routine nursing,and the observation group received extended operating room nursing combined with thermal insulation intervention based on four actions.The pain degree[visual analogue score(VAS)],recovery time of spontaneous breathing after surgery,recovery time,stay time in resuscitation room and the rate of adverse reactions were compared between the two groups.Results The VAS scores of the observation group at 1 hour,4 hours and 12 hours after surgery were lower than those of the control group(all P<0.05),but there was no statistical difference between the two groups at 24 hours after surgery(P>0.05).The recovery time of spontaneous respiration,recovery time and resuscitation room stay time in the observation group were shorter than those in the control group(all P<0.05).The rate of postoperative adverse reactions in the observation group was lower than that in the control group(3.33%vs.23.33%,P<0.05).The satisfaction of nursing in the observation group was higher than that in the control group(100.0%vs.83.33%,P<0.05).Conclusions Extended operating room nursing combined with insulation intervention based on four actions in patients with hysterectomy can reduce postoperative pain,accelerate postoperative recovery,improve surgical safety and overall nursing service quality.
