平肝养血汤对妊娠期高血压患者血压及血清因子水平的影响

The Influence of Liver-Pacifying and Blood-Nourishing Decoction on Blood Pressure and Serum Factor Levels of Hypertensive Patients During Pregnancy

目的:观察平肝养血汤对妊娠期高血压患者血压及血清因子水平的影响。方法:选取2021年1月至2022年6月上海市嘉定区安亭医院诊治的妊娠期高血压患者90例,按照随机数字表法分为对照组和研究组各45例。对照组给予常规西药治疗,并根据胎儿发育情况及患者血压控制情况,适时进行剖宫产,若患者病情不可控则需及时终止妊娠,研究组在对照组治疗的基础上给予平肝养血汤治疗。观察两组治疗前后中医证候积分、血压、血清因子[血管细胞间黏附分子-1(vascular cell adhesion molecule-1,VCAM-1)、白细胞介素(interleukin,IL)-4、IL-6、血管性血友病因子(von willebrand disease factor,vWF)、基质金属蛋白酶-9(matrix metalloproteinase-9,MMP-9)、血管内皮生长因子(vascular endothelial growth factor,VEGF)、转化生长因子-β_(1)(transforming growth factor-β_(1),TGF-β_(1))]变化情况及临床疗效。结果:研究组有效率为97.78%,对照组有效率为84.44%,研究组有效率高于对照组,差异有统计学意义(P<0.05);研究组治疗后头晕目眩、腰膝酸软、五心烦热、失眠多梦、胁痛、耳鸣健忘、咽喉干燥等中医证候评分均低于对照组,差异有统计学意义(P<0.05);研究组治疗后血压水平均低于对照组,差异有统计学意义(P<0.05);研究组治疗后VCAM-1、IL-6、vWF、MMP-9低于对照组,IL-4、VEGF、TGF-β_(1)高于对照组,差异有统计学意义(P<0.05)。结论:平肝养血汤能有效降低妊娠期高血压患者血压水平,缓解患者临床症状,调节血清因子水平。

Objective:To observe the influence of Liver-Pacifying and Blood-Nourishing Decoction on blood pressure and serum factor levels in hypertensive patients during pregnancy.Methods:A total of 90 patients with pregnancy-induced hypertension diagnosed and treated in Shanghai Jiading District Anting Hospital from January 2021 to June 2022 were selected and divided into the control group and the study group,with 45 cases in each group according to the random number table method.The control group was treated with conventional Western medicine,and the caesarean section was carried out timely according to the fetal development and the patient's blood pressure control.If the patient's condition was uncontrollable,the pregnancy should be terminated in time.The study group was treated with Liver-Pacifying and Blood-Nourishing Decoction on the basis of the control group.Then the changes in TCM syndrome score,blood pressure,serum factors[including vascular cell adhesion molecule-1(VCAM-1),interleukin(IL)-4,IL-6,von willebrand disease factor(vWF),matrix metalloproteinase-9(MMP-9)endothelial growth factor(MMP-9),vascular endothelial growth factor(VEGF),transforming growth factor-β_(1)(TGF-β_(1))]of the two groups before and after treatment and clinical efficacy were observed.Results:The effective rate of the study group was 97.78%,and that of the control group was 84.44%.The effective rate of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the TCM syndrome scores of the study group were lower than those of the control group,such as dizziness,soreness and weakness of waist and knees,vexing heat in chest,palms and soles,insomnia and dreaminess,hypochondriac pain,tinnitus and forgetfulness,and dry throat,and all the differences were statistically significant(P<0.05);The blood pressure level of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05);VCAM-1,IL-6,vWF and MMP-9 in the study group were lower than those in the control group,while IL-4,VEGF and TGF-β_(1)were higher than those in the control group,and all the differences were statistically significant(P<0.05).Conclusion:Liver-Pacifying and Blood-Nourishing Decoction can effectively reduce the blood pressure level of hypertensive patients during pregnancy,relieve their clinical symptoms,and regulate the levels of serum factors.
