超声二维斑点追踪在疑似冠心病患者中的诊断价值

Diagnostic value of two-dimensional speckle tracking in patients with suspected coronary heart disease

目的分析斑点追踪对冠心病的诊断价值。方法选取2020年1月至2021年5月赣州市人民医院收治的60例疑似冠心病患者为研究对象,给予患者超声二维斑点追踪技术检查,并以冠脉造影术(CAG)作为金标准,分析斑点追踪技术的诊断价值。同时按照CAG诊断的冠状动脉直径狭窄程度分为五组,分别为正常组、轻度狭窄、中度狭窄、重度狭窄及完全闭塞组,比较不同组别患者心脏功能情况。结果超声二维斑点追踪技术诊断灵敏度(97.83%)、特异度(91.67%)及准确度(93.33%),均高于常规超声心动图的72.92%、58.33%、70.00%,差异有统计学意义(P<0.05)。中、重度狭窄及闭塞组患者内心肌峰值纵向、整体纵向应变、左室整体环向应变表达低于正常、轻度狭窄,重度狭窄及闭塞组患者径向、圆周应变表达低于其他三组,差异有统计学意义(P<0.05)。结论超声二维斑点追踪技术对冠心病患者有较高诊断价值,定量评价患者左心室整体运动功能具有较高临床意义。

Objective To analyze the value of speckle tracking in coronary heart disease.Methods A total of 60 patients with suspected coronary heart disease treated in Ganzhou People's Hospital from January 2020 to May 2021 were examined by ultrasonic two-dimensional speckle tracking technology,and the diagnostic value of speckle tracking technology was analyzed with coronary angiography(CAG)as the gold standard.At the same time,according to the degree of coronary artery diameter stenosis diagnosed by CAG,they were divided into five groups:normal group,mild stenosis,moderate stenosis,severe stenosis and complete occlusion group.The cardiac function of patients in different groups was compared.Results The diagnostic sensitivity,specificity and accuracy of two-dimensional ultrasonic spot tracking technique were 97.83%,91.67%and 93.33%,which were higher than that of conventional echocardiography(72.92%,58.33%and 70.0%),and the differences were statistically significant(P<0.05).The expressions of longitudinal strain,global longitudinal strain and global circumferential strain in patients with moderate and severe stenosis and occlusion were lower than those in normal and mild stenosis,and the expressions of radial strain and circumferential strain in patients with severe stenosis and occlusion were lower than those in the other three groups(P<0.05).Conclusion Ultrasound two-dimensional speckle tracking technique has high diagnostic value in patients with coronary heart disease.It has high clinical significance to quantitatively evaluate the overall left ventricular motor function.
