血必净注射液、木防己汤联合西药治疗痰瘀互结型慢性阻塞性肺疾病急性加重期的效果

Effect of Xuebijing Injection,Mufangji Decoction Combined with Western Medicine on Acute Exacerbation of Phlegm-stasis Interjunction Chronic Obstructive Pulmonary Disease

目的:探究血必净注射液、木防己汤联合西药治疗痰瘀互结型慢性阻塞性肺疾病急性加重(AECOPD)患者的效果。方法:选取2021年4月—2022年4月菏泽市中医医院收治的痰瘀互结型AECOPD患者60例为观察对象,随机分为对照组和观察组,各30例。对照组给予常规西药治疗,观察组在对照组基础上应用血必净注射液联合木防己汤加味治疗,比较两组症状及体征积分改善情况、外周血以及血清相关指标改善情况、临床疗效。结果:治疗后,观察组咳嗽、咳痰、喘息及肺部啰音积分低于对照组,差异有统计学意义(P<0.05);治疗后,观察组外周血白细胞计数、血清降钙素原、超敏C反应蛋白、红细胞沉降率、白细胞介素-6水平低于对照组,差异有统计学意义(P<0.001);观察组治疗有效率高于对照组,差异有统计学意义(P=0.037)。结论:在痰瘀互结型慢性阻塞性肺疾病急性加重期患者中应用血必净注射液、木防己汤联合西药价值较高,可以改善临床症状及体征,缓解炎性反应,减少感染,改善肺功能,可在临床广泛应用。

Objective:To explore the effect of Xuebijing injection,Mufangji decoction combined with western medicine on acute exacerbation of phlegm-stasis interjunction chronic obstructive pulmonary disease(AECOPD).Methods:Sixty patients with AECOPD with phlegm-stasis interjunction treated in Heze Hospital of Traditional Chinese Medicine from April 2021 to April 2022 were selected as the observation objects,and randomly divided into control group and observation group,with thirty cases in each group.The control group was treated with conventional Western medicine,and the observation group was treated with Xuebijing injection combined with Mufangji decoction on the basis of the control group.The improvement of symptom and sign scores,peripheral blood and serum related indexes and clinical efficacy were compared between the two groups.Results:After treatment,the scores of cough,sputum,wheezing and pulmonary rales in observation group were lower than those in control group,and the differences were statistically significant(P<0.05);After treatment,the peripheral blood white blood cell count,serum procalcitonin,hypersensitive C-reactive protein,erythrocyte sedimentation rate and interleukin-6 level in observation group were lower than those in control group,with statistical significance(P<0.001);The effective rate of treatment in the observation group was higher than that in the control group,the difference was statistically significant(P=0.037).Conclusion:Xuebijing injection and Mufengji decoction combined with Western medicine have high value in patients with acute exacerbation of phlegm-stasis interjunction chronic obstructive pulmonary disease,which can improve clinical symptoms and signs,alleviate inflammatory response,reduce infection and improve lung function,and can be widely used in clinical practice.
