甲状腺激素在甲状腺疾病中的诊断价值

Diagnostic Value of Thyroid Hormones in Thyroid Diseases

目的:探讨甲状腺激素对甲状腺疾病的诊断价值。方法:选择2021年2月—2023年3月建湖县第二人民医院收治的甲状腺疾病患者162例为研究对象,按不同疾病类型分为甲状腺功能亢进组(甲亢组)与甲状腺功能减退组(甲减组),各81例;选择同期入院体检的健康体检者81例设为健康对照组。对三组进行T3、T4、促甲状腺激素(TSH)检测,对比检测结果,评估3项指标单独检测和联合检测的诊断效能。结果:甲亢组T3、T4水平高于健康对照组,TSH水平低于健康对照组,差异有统计学意义(P<0.001);甲减组T3、T4水平低于健康对照组,且TSH水平高于健康对照组,差异有统计学意义(P<0.001);T3、T4、TSH联合检测诊断甲状腺功能亢进、减退敏感性和特异性均高于单独检测,差异有统计学意义(P<0.05)。结论:T3、T4、TSH联合检测能准确反映甲状腺功能状况,对诊断甲状腺疾病有指导价值,值得应用。

Objective:To explore the diagnostic value of thyroid hormones on thyroid diseases.Methods:One hundred and sixty-two patients with thyroid diseases admitted to Jianhu County Second People's Hospital from February 2021 to March 2023 were selected for the study.According to different disease types,they were divided into hyperthyroidism group(hyperthyroidism group)and hypothyroidism group(hypothyroidism group),eighty-one cases each.Eighty-one healthy patients who were admitted to the hospital for physical examination at the same time were selected as the healthy control group.T3,T4,and thyroid stimulating hormone(TSH)were measured in the three groups,and the results were compared to evaluate the diagnostic efficacy of the three indexes alone and in combination.Results:The levels of T3 and T4 in the hyperthyroidism group were higher than those in the healthy control group,and the levels of TSH were lower than those in the healthy control group,and the differences were statistically significant(P<0.001);In the hypothyroidism group,T3 and T4 levels were lower than those in the healthy control group,and TSH levels were higher than those in the healthy control group,with statistically significant differences(P<0.001);The sensitivity and specificity of combined T3,T4 and TSH detection for diagnosing hyperthyroidism and hypothyroidism were higher than that of individual detection,and the difference was statistically significant(P<0.05).Conclusion:Combined T3,T4 and TSH test can accurately reflect the thyroid function status,which has guiding value for diagnosing thyroid diseases and is worth applying.
