摘要
目的分析2019年—2022年上海市闵行区空肠弯曲菌分子分型与耐药性特征,建立空肠弯曲菌分子和药敏数据库,为食源性疾病暴发追踪溯源提供基线数据。方法脉冲场凝胶电泳(PFGE)对空肠弯曲菌进行分子分型,利用BioNumerics软件进行聚类分析,建立分子分型数据库,同时用肉汤稀释法测定菌株对7大类11种抗菌药物的敏感性。结果收集腹泻患者粪便标本3995份,空肠弯曲菌的检出率为1.4%(55/3995);采集生禽肉样本240份,空肠弯曲菌检出率为6.3%(15/240),其中鸡肉中检出率为3.3%(8/240)、鸭肉检出率为1.3%(3/240);鸽子和鹅肉检出率均为0.8%(2/240)。对腹泻患者及生禽肉来源的共70株菌进行PFGE分子分型,58株成功分为25个型别,其中36株菌株对11种抗生素的耐药谱分为21种。28株空肠弯曲菌对3类及以上的抗生素耐药。结论上海市闵行区空肠弯曲菌分子型别成多样化和复杂化特征,引起的腹泻病例以散发为主;生禽肉中也存在空肠弯曲菌的污染,应该加强食品及其销售环节的监督,减少空肠弯曲菌的污染。
Objective This paper aims to systematically analyze the characteristics of molecular types and drug resistance of Campylobacter jejuni in Minhang District of Shanghai from 2019 to 2022 and to establish a molecular and drug sensitivity database of Campylobacter jejuni,so as to provide baseline data for tracing the source of food-borne disease outbreaks.Methods The molecular typing of Campylobacter jejuni was performed by pulsed field gel electrophoresis gel electrophoresis(PFGE),and the database of molecular typing was established by clustering analysis with BioNumerics software,and the sensitivity of Campylobacter jejuni to 11 kinds of 7 types of antibiotics was determined by broth dilution method.Results The detection rate of Campylobacter jejuni was 1.4%(55/3995)in 3995 stool samples collected from diarrhea patients.The detection rate of Campylobacter jejuni was 6.3%(15/240)in 240 raw poultry samples,3.3%(8/240)in chicken meat and 1.3%(3/240)in duck meat.The detection rates of pigeon and goose meat were both 0.8%(2/240).A total of 70 strains from diarrhea patients and raw poultry were classified by PFGE molecular typing.58 strains were classified into 25 types,and 36 strains were classified into 21 types which were resistant to 11 antibiotics.28 strains of Campylobacter jejuni were resistant to three or more types of antibiotics.A total of 70 strains from diarrhea patients and raw poultry meat were typed by PFGE,and 58 strains were divided into 25 types.Conclusion The molecular types of Campylobacter jejuni in Minhang District of Shanghai were diverse and complicated,and the diarrhea cases were mainly sporadic.Campylobacter jejuni contamination also exists in raw poultry meat.Supervision of food and its sales should be strengthened to reduce the contamination of Campylobacter jejuni.
