杭州市医务人员手卫生消毒质量影响因素调研分析

Investigation and study on influencing factors of hand hygiene quality for medical staff in Hangzhou

目的调研分析医务人员手卫生消毒质量的影响因素,以期提高医务人员手卫生质量,预防与控制医院感染、保障患者和医务人员安全。方法随机选取杭州市各级医疗机构医务人员128例,采用问卷调查和实验室检测方法对医务人员手卫生现况进行调查。结果128例医务人员手卫生消毒质量合格数为93例,合格率为72.7%。单因素分析结果显示,不同性别、岗位、学历、医学背景、手卫生步骤、免洗手消毒剂有效成分的医务人员手卫生消毒质量差异均无统计学意义(P>0.05),不同培训频次和不同免洗手消毒剂使用量的医务人员手卫生消毒质量差异有统计学意义(P<0.05)。多因素logistic回归分析结果显示,培训1次/半年和免洗手消毒剂使用量<3 ml是医务人员手卫生消毒质量的危险性因素(P<0.05)。结论医务人员在执行卫生手过程中不仅应加强培训频次和质量,还应重视手卫生消毒过程中免洗手消毒剂的规范使用。

Objective This study aims to investigate and analyze the influencing factors of hand hygiene and disinfection quality,in order to improve the hand hygiene quality for medical staff,prevent and control nosocomial infection,and ensure the safety of patients and medical staff.Methods A total of 128 medical staff were randomly selected from medical institutions in Hangzhou,and questionnaire survey and laboratory testing were used to investigate the hand hygiene status of medical staff.Results The qualified number of hand hygiene quality of 128 medical staff was 93,and the qualified rate was 72.7%.The results of univariate analysis showed that there was no statistical significance on the difference in the quality of hand hygiene disinfection among medical staff of different gender,post,education background,medical background,hand hygiene steps,effective components of hand sanitizer(P>0.05),but there was statistical significance on the difference in the quality of hand hygiene disinfection among medical staff of different training frequency and different doses of hand sanitizer(P<0.05).The results of multivariate logistic regression analysis showed that training once half year and the use of disposable hand sanitizer<3 ml are risk factors for the hand hygiene quality of medical staff(P<0.05).Conclusion Medical staff should not only strengthen training frequency and quality,but also pay attention to the standard use of hand sanitizer in the process of hand hygiene.
