摘要
目的观察比较口腔种植修复术与固定义齿修复术的疗效,及对患者咀嚼功能、IL-6、TNF-α的影响,并分析IL-6、TNF-α与患者咀嚼功能的相关性。方法选取2019年1月—2022年6月在口腔科就诊的牙列缺损患者80例并分成对照组和观察组,对照组患者治疗采用固定义齿修复法,观察组患者治疗采用口腔种植修复法。观察患者治疗前后龈沟液IL-6、TNF-α含量及咬合力水平,对患者治疗后进行疗效评价。结果2组患者治疗前IL-6、TNF-α、咬合力水平无明显差异(P>0.05),治疗后2组患者IL-6、TNF-α、咬合力水平都升高,且观察组患者IL-6、TNF-α水平明显低于对照组患者(P<0.05),观察组患者咬合力明显高于对照组患者(P<0.05);观察组患者疗效优良率明显高于对照组患者(P<0.05);观察组患者IL-6、TNF-α水平均与患者咬合力存在明显负相关(r=-0.437,r=-0.460,P<0.05)。结论牙列缺损口腔种植修复法临床疗效更好、能明显改善患者咀嚼功能,并且其疗效与减轻患者炎症反应、改善患者口腔微环境有关。
Objective This paper aims to observe and compare the efficacy of dental implant repair and fixed denture repair,and analyze the effect on masticatory function,IL-6,TNF-α,and the correlation between IL-6,TNF-α and masticatory function.Methods A total of 80 patients with dentition defect treated in stomatology department from June 2019 to June 2022 were selected and divided into control group and observation group.The patients in the control group were treated with fixed denture repair,and the patients in the observation group were treated with oral implant repair.The bite force and the levels of IL-6 and TNF-α in gingival crevicular fluid were observed before and after treatment,and the curative effect of patients after treatment was evaluated.Results The levels of IL-6,TNF-α and the bite force of the two groups were not significantly different before treatment(P>0.05),but the levels of IL-6,TNF-α and bite force of the two groups were increased after treatment,and the levels of IL-6 and TNF-α of the observation group were lower than those of the control group(P<0.05).The biting force of observation group was higher than that of control group(P<0.05).The curative effect rate of observation group was higher than that of control group(P<0.05).The levels of IL-6 and TNF-α in observation group were negatively correlated with the biting force of patients(r=-0.437,r=-0.460,P<0.05).Conclusion Dental implant repair for dentition defect has better clinical effect and can significantly improve patients'masticatory function,and its effect is related to reducing patients'inflammatory reaction and improving patients'oral microenvironment.
