摘要
目的探讨口腔种植体周围炎与白细胞介素(IL)-17A、IL-17A-197G/A基因多态性、肿瘤坏死因子(TNF)-α的相关性。方法选取2018年1月—2022年8月医院收治的60例口腔种植体周围炎患者(观察组)及同期口腔种植修复且无种植体周围感染者60例(对照组),采集2组患者龈沟液标本,测定2组IL-17A及TNF-α水平,以实时荧光PCR法测定IL-17A-197G/A基因多态性。结果60例种植体周围炎患者共分离鉴定出病原菌112株,包括76株厌氧菌(67.86%),32需氧菌(28.57%),4株有益菌(3.57%)。观察组黏膜肿胀程度、探诊深度、附着丧失度及牙龈出血指数均高于对照组(P<0.05)。观察组患者龈沟液IL-17A及TNF-α水平均高于对照组,IL-17A-197G/A基因多态性AA基因型高于对照组,A等位基因频率高于对照组(P<0.05)。采用Spearman相关性分析,IL-17A、TNF-α、IL-17A-197G/A AA基因型、A等位基因与黏膜肿胀程度、探诊深度、附着丧失度及牙龈出血指数成正相关关系。结论口腔种植体周围炎患者龈沟液IL-17A、TNF-α水平以及IL-17A-197G/A位点AA基因型、A等位基因频率明显升高,与口腔种植体周围炎有显著相关性。
Objective This paper aims to investigate the association of oral periimplantitis with interleukin(IL)-17A,IL-17A-197G/A gene polymorphism and tumor necrosis factor(TNF)-α.Methods From January 2018 to August 2022,60 patients with oral periimplantitis admitted to the hospital(observation group)and 60 patients with oral implant restoration without periimplantitis infection(control group)were selected.The gingival crevicular fluid samples of the two groups were collected,and the levels of IL-17A and TNF-α were measured.The polymorphism of IL-17A-197G/A gene was determined by real-time PCR.Results A total of 112 strains of pathogenic bacteria were isolated from 60 patients with periimplantitis,including 76 anaerobic bacteria(67.86%),32 aerobic bacteria(28.57%)and 4 beneficial bacteria(3.57%).The degree of mucosal swelling,probing depth,attachment loss and gingival bleeding index in the observation group were higher than those in the control group(P<0.05).The levels of IL-17A and TNF-α in gingival crevicular fluid of the observation group were higher than those of the control group,the AA genotype of IL-17A-197G/A gene polymorphism was higher than that of the control group,and the frequency of A allele was higher than that of the control group(P<0.05).According to Spearman correlation analysis,IL-17A,TNF-α,AA genotype and A allele of IL-17A-197G/A were positively correlated with the degree of mucosal swelling,depth of probing,loss of attachment and gingival bleeding index.Conclusion The levels of IL-17A and TNF-α in gingival crevicular fluid and the frequencies of AA genotype and A allele of IL-17A-197G/A locus are significantly increased in patients with oral periimplantitis,which are significantly correlated with oral periimplantitis.
