芪黄健脾滋肾颗粒对系统性红斑狼疮患者中性粒细胞/淋巴细胞、纤维蛋白原/前白蛋白的影响

Effects of Qihuang Jianpi Zishen Granules on neutrophil/lymphocyte ratio and fibrinogen/prealbumin ratio in patients with systemic lupus erythematosus

目的:观察芪黄健脾滋肾颗粒对系统性红斑狼疮(SLE)患者的临床疗效及对中性粒细胞/淋巴细胞(NLR)、纤维蛋白原/前白蛋白(FPR)等的影响。方法:选取60例SLE患者,随机分成对照组和观察组,每组各30例,对照组采用西医常规基础治疗,泼尼松片(或等效剂量的其他激素)最小使用剂量为5 mg/d,最大使用剂量为40 mg/d,用量依据病情进行调整;硫酸羟氯喹片,0.1 g/片,口服0.1 g~0.2 g/次,2次/d。观察组在对照组治疗基础上加用芪黄健脾滋肾颗粒。4周为1个疗程。4周后比较两组患者中医证候总积分、系统性红斑狼疮疾病活动度评分(SLEDAI)、中性粒细胞/淋巴细胞、纤维蛋白原/前白蛋白、血沉(ESR)、C反应蛋白(CRP)、免疫球蛋白G(IgG)、补体C3、C4水平及临床疗效。结果:4周后,观察组中医证候总积分、SLEDAI评分、NLR、FPR、ESR、CRP、IgG等指标低于对照组,C3、C4水平高于对照组(P<0.05,P<0.01)。结论:西药联合芪黄健脾滋肾颗粒治疗SLE具有较好的应用前景。

Objective:To observe the clinical efficacy of Qihuang Jianpi Zishen Granules in the treatment of systemic lupus erythematosus(SLE)and the effects on neutrophil/lymphocyte ratio(NLR)and fibrinogen/prealbumin ratio(FPR).Methods:60 patients with SLE were randomly divided into control group and observation group,30 cases in each group.The patients in the control group were treated with conventional basic treatment of western medicine.The minimum dosage of prednisone tablets(or equivalent doses of other hormones)was 5 mg/d,and the maximum dosage was 40 mg/d.The dosage was adjusted according to the condition of the patient.The usage and dosage of hydroxychloroquine sulfate tablets were oral 0.1 g~0.2 g/time,twice daily.The patients in the observation group were treated with Qihuang Jianpi Zishen Granules on the treatment basis of the control group.One course of treatment was 4 weeks.After 4 weeks,the total TCM syndrome scores,systemic lupus erythematosus disease activity index(SLEDAI),neutrophil/lymphocyte(NLR),fibrinogen/prealbumin(FPR),erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),immunoglobulin G(IgG),complement C3 and C4 levels and clinical efficacy were compared between the two groups.Results:After 4 weeks,the total TCM syndrome scores,SLEDAI,NLR,FPR,ESR,CRP,IgG and other indicators of the observation group were lower than those of the control group,and the C3 and C4 levels were higher than those of the control group(P<0.05,P<0.01).Conclusion:Western medicine combined with Qihuang Jianpi Zishen Granules has a good application prospect in the treatment of SLE.
