八段锦联合温针灸治疗风寒痹阻型神经根型颈椎病随机对照研究

Randomized controlled trial of Baduanjin combined with warm acupuncture in the treatment of cervical spondylotic radiculopathy caused by wind-cold obstruction

目的:探讨八段锦功法锻炼联合温针灸治疗风寒痹阻型神经根型颈椎病的临床疗效及对血清中肿瘤坏死因子-α(TNF-α)、白介素-1β(IL-1β)和白介素-6(IL-6)含量的影响。方法:将符合纳入标准的80例风寒痹阻型神经根型颈椎病患者随机分为对照组和观察组,每组40例。对照组采用颈部肌群等长抗阻训练联合温针灸治疗,观察组采用八段锦功法锻炼联合温针灸治疗,疗程均为4周。观察两组患者治疗前和治疗4周后简化McGill疼痛量表(SFMPQ)评分、麻木程度评分、颈椎功能障碍指数(NDI)评分及血清中TNF-α、IL-1β和IL-6含量,比较两组临床疗效的差异。结果:与本组治疗前比较,两组患者SF-MPQ评分、麻木程度评分、NDI评分及血清中TNF-α、IL-1β、IL-6含量均下降(P<0.05)。治疗4周后,观察组SF-MPQ评分、麻木程度评分、NDI评分及血清中TNF-α、IL-1β、IL-6含量低于对照组(P<0.05)。对照组总有效率为78.95%,观察组总有效率为94.74%,观察组总有效率优于对照组(P<0.05)。结论:八段锦功法具有动作缓慢柔和、运动强度适中等特点,适合神经根型颈椎病患者锻炼。八段锦联合温针灸通过减轻炎性反应而改善风寒痹阻型神经根型颈椎病患者的临床症状。

Objective:To explore the clinical efficacy of Baduanjin exercise combined with warm acupuncture in the treatment of cervical spondylotic radiculopathy caused by wind-cold obstruction and its influence on the content of tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)and interleukin-6(IL-6)in serum.Methods:80 patients with cervical spondylotic radiculopathy caused by wind-cold obstruction who met the inclusion criteria were randomly divided into control group and observation group,with 40 cases in each group.The patients in the control group were treated with isometric resistance training of neck muscles combined with warm acupuncture,and the patients in the observation group were treated with Baduanjin exercise combined with warm acupuncture.The course of treatment was 4 weeks.The short-form McGill pain questionnaire(SF-MPQ)score,numbness degree score,neck disability index(NDI)score and serum TNF-α,IL-1βand IL-6 contents of the two groups were observed before treatment and 4 weeks after treatment,and the clinical effects were compared between the two groups.Results:Compared with those before treatment,the SF-MPQ score,numbness degree score,NDI score and serum TNF-α,IL-1β,IL-6 contents in the two groups were decreased(P<0.05).After 4 weeks of treatment,the SFMPQ score,numbness degree score,NDI score and serum TNF-α,IL-1β,IL-6 contents in the observation group were lower than those in the control group(P<0.05).The total effective rate was 78.95%in control group and 94.74%in observation group.The total effective rate of the observation group was better than that of the control group(P<0.05).Conclusion:Baduanjin exercise has the characteristics of slow and gentle movement and moderate exercise intensity,which is suitable for patients with cervical spondylotic radiculopathy.Baduanjin combined with warm acupuncture can improve the clinical symptoms of cervical spondylotic radiculopathy caused by wind-cold obstruction by reducing inflammatory response.
