高脂血症性急性胰腺炎复发危险因素的研究

Study on recurrence risk factors of hyperlipidemic acute pancreatitis

目的预测高脂血症性急性胰腺炎复发的危险因素。方法选择2017年1月1日至2021年12月31日昆明医科大学第二附属医院确诊为高脂血症性急性胰腺炎的136例患者作为研究对象。其中49例患者复发,纳入复发组,将剩余未复发87例患者纳入非复发组。收集两组患者的一般资料、慢性病史、并发症和实验室指标,并进行单因素分析,对于单因素分析结果有意义的部分再进行Logistic多因素回归分析,进一步确定高脂血症性急性胰腺炎复发的危险因素。结果单因素分析结果显示,BMI、住院天数、病情严重程度、APACHEⅡ评分、痛风、SIRS、MODS、中性粒细胞、超敏C反应蛋白、血钙和肌酐与高脂血症性急性胰腺炎复发密切相关;多因素Logistic回归分析发现,BMI、APACHEⅡ评分、超敏C反应蛋白、SIRS和病情严重程度是高脂血症性急性胰腺炎复发的独立危险因素。结论BMI、APACHEⅡ评分、超敏C反应蛋白、SIRS和病情严重程度是高脂血症性急性胰腺炎复发的独立危险因素。

Objective To predict the risk factors of recurrence of hyperlipidemic acute pancreatitis(HAP).Methods A total of 136 patients with HAP diagnosed in the Second Affiliated Hospital of Kunming Medical University from Jan.1st,2017 to Dec.31st,2021 were selected as subjects.Among them,49 recurrent patients were included in the recurrent group,and 87 patients without recurrence were included in the non-recurrent group.The general data,chronic history,complications and laboratory indexes of the two groups were collected.Univariate analysis was performed.For the significant part of univariate analysis,Logistic multiple regression analysis was used to further determine the risk factors of HAP recurrence.Results Univariate analysis showed that BMI,length of hospital stay,severity of illness,APACHEⅡscore,gout,SIRS,MODS,neutrophils,hypersensitive C-reactive protein,serum calcium and creatinine were closely related to HAP recurrence.While multivariate Logistic regression analysis showed that BMI,APACHEⅡscore,hypersensitive C-reactive protein,SIRS and disease severity were independent risk factors for HAP recurrence.Conclusion BMI,APACHEⅡscore,hypersensitive C-reactive protein,SIRS and severity of illness are independent risk factors for HAP recurrence.
