序贯针灸联合补肾活血方治疗肾虚血瘀型多囊卵巢综合征的临床效果观察

Clinical effect of sequential acupuncture combined with Bushen Huoxue Decoction in treating polycystic ovary syndrome of kidney deficiency and blood stasis type

目的探讨序贯针灸联合补肾活血方治疗肾虚血瘀型多囊卵巢综合征(PCOS)的临床效果。方法选取2020年10月至2022年2月南通市中医院妇科门诊收治的60例肾虚血瘀型PCOS患者作为研究对象,依据随机数字法分为对照组和序贯针灸联合治疗组,各30例。对照组在PCOS常规治疗基础上给予自拟补肾活血方治疗,序贯针灸联合治疗组在对照组治疗基础上联合序贯针灸治疗方案。比较两组月经及中医证候积分、子宫内膜厚度与卵巢体积、血清激素水平、妊娠率,并对安全性进行评估。结果干预后,与对照组相比,序贯针灸联合治疗组月经及中医证候积分各项评分均显著降低(P<0.05);序贯针灸联合治疗组的子宫内膜厚度显著高于对照组,卵巢体积显著小于对照组(P<0.05);与对照组相比,序贯针灸联合治疗组卵泡刺激素(FSH)及黄体生成素(LH)水平显著降低,而雌二醇(E2)水平显著升高(P<0.05);序贯针灸联合治疗组妊娠率显著高于对照组(46.67%vs.20.00%,P<0.05);两组均未见严重不良反应。结论基于自拟补肾活血方的序贯针灸治疗方案治疗肾虚血瘀型PCOS的临床效果显著,可有效提高肾虚血瘀型PCOS患者的中医证候积分、改善子宫内膜厚度和卵巢体积,对血清激素水平调节作用明显,并可有效提高妊娠率。

Objective To explore the clinical effect of sequential acupuncture combined with Bushen Huoxue Decoction on polycystic ovary syndrome(PCOS)of kidney deficiency and blood stasis type.Methods A total of 60 patients with PCOS of kidney deficiency and blood stasis type in Nantong Hospital of Traditional Chinese Medicine were selected as study subjects and divided into control group(routine PCOS treatment with self-made formula for Bushen Huoxue Decoction)and sequential acupuncture combined treatment group(control group combined with sequential acupuncture treatment)according to random number method,with 30 cases in each group.The menstrual correlation and traditional Chinese medicine(TCM)syndrome score,endometrial thickness and ovarian volume,serum hormone level and pregnancy rate were compared between the groups.Finally,the safety was evaluated.Results Compared with the control group,the scores of menstruation and TCM syndrome scores in the sequential acupuncture combined treatment group were significantly lower(P0.05).The endometrial thickness of the sequential acupuncture combined treatment group was significantly higher than that of the control group,and the ovarian volume was significantly lower than that of the control group(P0.05).Compared with the control group after the intervention,the levels of follicle stimulating hormone(FSH)and luteinizing hormone(LH)in the sequential acupuncture combined treatment group were significantly decreased,while estradiol(E_2)was significantly increased(P0.05).The pregnancy rate of the sequential acupuncture combined treatment group was significantly higher than that of the control group(46.67%vs.20.00%,P0.05),and there were no serious adverse reactions in both groups.Conclusions The sequential acupuncture treatment scheme based on the self-made formula for Bushen Huoxue Decoction has a significant clinical effect in treating PCOS of kidney deficiency and blood stasis type,which can effectively improve the TCM syndrome score of patients with PCOS of kidney deficiency and blood stasis type,improve the endometrial thickness and ovarian volume,significantly regulate the level of serum hormone and effectively improve the pregnancy rate.
