超声引导联合腔内心电图技术在PICC置管尖端定位中的应用

摘要 目的 研究超声引导联合腔内心电图技术在经外周置入中心静脉导管(peripherally inserted central catheter,PICC)术置管尖端定位中的应用效果。方法 选取2022年1月—2022年12月于茂名市中医院行PICC置管的75例患者作为研究对象,将其按照随机数字表法分为对照组(n=37)与观察组(n=38)。两组患者分别实施超声引导PICC置管、超声引导联合腔内心电图技术PICC置管,将两组的穿刺成功率(首次穿刺成功率、5 min内穿刺成功率)、穿刺用时、穿刺时疼痛程度、置管准确率(一次置管成功率、尖端到位率)、患者满意度等指标进行组间比较分析。结果 (1)穿刺成功率:观察组初次穿刺成功率明显高于对照组,5 min内穿刺成功率超过对照组,差异有统计学意义(P <0.05)。(2)穿刺时间与疼痛感受:观察组患者穿刺时间较短,且感受到的疼痛程度低于对照组,差异有统计学意义(P <0.05)。(3)置管精度:观察组患者一次成功置管的比例高于对照组,置管准确位置接近于顶尖水平,差异有统计学意义(P <0.05)。(4)患者满意度:观察组患者的满意度高于对照组,差异有统计学意义(P<0.05)。结论 在使用超声引导与腔内心电图检查技术协同作用进行PICC置管顶尖精准定位时,可以提高穿刺成功率和置管精度,同时可缩短穿刺用时,缓解患者的疼痛感,且患者的整体满意度较高。 Objective To study the application effect of ultrasonic guidance combined with intracavitary electrocardiogram in(peripherally inserted central catheter,PICC) catheter tip location.Methods A total of 75 patients who received PICC catheterization in Maoming Hospital of Traditional Chinese Medicine from January 2022 to December 2022 were selected as research objects,and they were divided into control group(n = 37) and observation group(n = 38) by random number table method.Patients in the two groups were respectively subjected to ultrasound-guided PICC catheterization and ultrasound-guided PICC catheterization combined with endovascular ECG technology.The success rate of puncture(success rate of first puncture,success rate of puncture within 5 minutes),puncture time,pain degree during puncture,catheterization accuracy(success rate of one catheterization,tip reaching rate),patient satisfaction and other indicators of the two groups were compared and analyzed.Results(1)Puncture success rate:the first puncture success rate in the observation group was higher than that in the control group,and the success rate within 5 minutes was higher than that in the control group,with statistical significance(P<0.05).(2)Puncture time and pain degree:the puncture time of observation group was shorter than that of control group,and the pain degree score of patients during puncture was lower than that of control group,with statistical significance(P<0.05).(3) Catheterization accuracy:the success rate of one-time catheterization in observation group was higher than that in control group,and the tip location rate was higher than that in control group,with statistical significance(P<0.05).(4)Patient satisfaction:after the evaluation of satisfaction,the satisfaction of patients in the observation group was greater than that in the control group,with statistical significance(P<0.05).Conclusion The application of ultrasound guidance combined with intracavitary electrocardiogram technology in PICC catheterization tip positioning can improve the success rate of puncture and catheterization accuracy,shorten the puncture time,relieve patients' pain,and increase patients' overall satisfaction,so it has reference significance and further promotion value.
