2018—2022年福州市第一医院中药饮片应用情况分析

Analysis on the Application of Chinese Medicine Decoction Pieces in the First Hospital of Fuzhou From 2018 to 2022

目的 分析2018—2022年福州市第一医院门诊中药饮片的应用情况,为制定中药饮片采购计划、合理用药管理提供依据。方法 从医院系统中导出近5年门诊中药饮片处方数量、处方剂数、销售金额、消耗量、科室用药情况等数据,进行分类统计分析。结果 2018年1月—2022年12月,门诊中药饮片处方数量、处方剂数和销售金额总体呈下降趋势。消耗量排名在前10位的中药饮片品种总体变动不大,黄芪、茯苓、白芍、党参、枸杞子和生地黄这6味中药每年消耗量均在前10位。按功效分类消耗量较大品种有补虚药、利水渗湿药和清热药。中药饮片在中西医肿瘤科和中医科这两个科室应用最多,远高于其他科室。结论 近年来福州市第一医院门诊中药饮片的使用在下降;制定采购计划时应结合医生名单变动和社会环境变化等进行调整,多购进消耗量较大的补虚药、利水渗湿药和清热药等,以保障临床合理用药的需求;但也要注意用量大药品的合理用药,防止中药品质变异造成损失。中西医肿瘤科和中医科是医院中药饮片应用最多的科室,其他科室要多与中医药类科室合作,探索开展中西医结合诊疗模式,同时积极引进中医药人才,有利于推动中医药事业在综合性医院的发展。

Objective To analyze the application of Chinese medicine decoction pieces in the outpatient department of the First Hospital of Fuzhou from 2018 to 2022,and to provide basis for the formulation of Chinese medicine decoction pieces procurement plan and rational drug use management.Methods Data such as the number of Chinese medicine decoction pieces prescriptions,the number of prescriptions,the consumption amount,the consumption amount and the drug use in departments in the outpatient department in recent 5 years were derived from the hospital system for classified statistical analysis.Results From January 2018 to December 2022,the number of prescriptions,the number of prescriptions and the consumption sum of Chinese medicine decoction pieces in outpatient department showed a decreasing trend.The top 10 Chinese medicine decoction pieces in terms of consumption did not change much on the whole,and the 6 Chinese medicine decoction pieces in terms of consumption of radix astragalus,poria cocos,radix paeoniae alba,radix codonopsis codonopsis,wolfberry and radix rehmanniae were all in the top 10.According to the classification of efficacy,there was high consumption of drugs for supplementing deficiency,improving water and dampness,and clearing heat.Chinese medicine decoction pieces were used most in the oncology department of traditional Chinese and western medicine and the department of traditional Chinese medicine,which was much higher than other departments.Conclusion In recent years,the use of Chinese medicine decoction pieces in outpatient department of the First Hospital of Fuzhou has been declining,so the procurement plan should be adjusted according to the changes of doctor list and social environment.To ensure the clinical rational use of drugs,we should also pay attention to the rational use of drugs with large quantities to prevent the loss caused by quality variation of traditional Chinese medicine.The oncology department of traditional Chinese and western medicine and the department of traditional Chinese medicine are the most applied departments of Chinese medicine decoction pieces in hospital.Other departments should cooperate more with the department of traditional Chinese medicine,explore the diagnosis and treatment mode of integrated traditional Chinese and western medicine,and actively introduce Chinese medicine talents,which is conducive to promoting the development of Chinese medicine in general hospital.
