高血压性脑出血患者长链非编码RNA锌指结构反义转录本1的表达与颅内动脉狭窄程度及预后的关系

摘要 目的 探讨高血压性脑出血(hypertensive intracerebral hemorrhage,HICH)患者长链非编码RNA(long non-coding RNA,LncRNA)锌指结构反义转录本1(zine finger antisense 1,ZFAS1)表达量与颅内动脉狭窄程度及复发性高血压性脑出血(recurrent hypertensive intracerebral hemorrhage,RHCH)的关系。方法 回顾性分析2014年2月至2020年2月河北省保定市第二中心医院收治的192例HICH患者的临床资料和随访资料,根据随访期间是否出现RHCH将研究对象分为RHCH组(出现RHCH,30例)和非RHCH组(未出现RHCH,162例)。比较分析两组患者的临床特征。采用Pearson相关系数法分析LncRNA ZFAS1表达量与颅内动脉狭窄程度的相关性。采用受试者工作特征曲线分析LncRNA ZFAS1表达量对RHCH的预测价值。采用多因素logistic回归方法分析RHCH的独立影响因素。结果 RHCH组患者的年龄、收缩压、血肿体积、颅内动脉狭窄程度、LncRNA ZFAS1表达量均显著大于或高于非RHCH组(P<0.05),格拉斯哥昏迷评分显著低于非RHCH组(P<0.05)。Pearson相关系数法分析结果显示,LncRNA ZFAS1表达量与颅内动脉狭窄程度呈显著正相关(r=0.615,P<0.001)。受试者工作特征曲线分析结果显示,LncRNA ZFAS1表达量预测RHCH的曲线下面积为0.881,敏感度为80.0%,特异度为82.0%。多因素logistic回归分析结果显示,年龄、收缩压、颅内动脉狭窄程度、格拉斯哥昏迷评分和LncRNA ZFAS1表达量是RHCH的独立影响因素(P<0.05)。结论 LncRNA ZFAS1表达量与颅内动脉狭窄程度和RHCH的发生相关,且对RHCH的发生具有一定预测价值。 Objective To investigate the relationship between the expression of LncRNA ZFAS1 and the degree of intracranial cerebral artery stenosis,recurrent hypertensive intracerebral hemorrhage(RHCH).Method The clinical data and follow-up data of 192 patients with HICH admitted to the Baoding Second Central Hospital from February 2014 to February 2020 were retrospectively analyzed.They were divided into RHCH group(30 cases with RHCH)and non-RHCH group(162 cases without RHCH)according to whether RHCH occurred during the follow-up period.The clinical features of the two groups were compared and analyzed.The correlation between LncRNA ZFAS1 expression and intracranial cerebral artery stenosis was analyzed by Pearson correlation coefficient.The predictive value of LncRNA ZFAS1 expression in RHCH was analyzed by ROC curve.Multiple logistic regression method was used to analyze the independent influencing factors of RHCH.Result Age,systolic blood pressure,hematoma volume,intracranial artery stenosis degree and LncRNA ZFAS1 expression in RHCH group were significantly higher than those in non-RHCH group(P<0.05),and GOS was significantly lower than that in non-RHCH group(P<0.05).The results of Pearson correlation coefficient analysis showed that LncRNA ZFAS1 expression was significantly positively correlated with the degree of intracranial cerebral artery stenosis(r=0.615,P<0.001).The ROC curve analysis showed that the area under the curve of LncRNA ZFAS1 expression predicting RHCH was 0.881,with the sensitivity of 80.0%,and the specificity of 82.0%.Multivariate logistic regression analysis showed that age,systolic blood pressure,intracranial cerebral artery stenosis degree,GOS and LncRNA ZFAS1 expression were independent factors for RHCH(P<0.05).Conclusion The expression level of LncRNA ZFAS1 is correlated with the degree of intracranial cerebral artery stenosis and the occurrence of RHCH,and has certain predictive value for the occurrence of RHCH.
