改良式翼状胬肉切除联合自体角膜缘干细胞移植术的远近期疗效研究

A Study on the far and short term efficacy of modified pterygium excision combined with autologous limbal stem cell transplantation for the treatment of pterygium

目的 探讨改良式翼状胬肉切除联合自体角膜缘干细胞移植术(CAT)治疗翼状胬肉的远近期疗效。方法 选取2021年6月至2023年1月在本院治疗的96例翼状胬肉患者临床资料,分为两组各48例。对照组接受改良翼状胬肉切除治疗,观察组接受改良翼状胬肉切除+CAT治疗。比较两组治疗效果,记录治疗前后视力质量、泪膜功能[泪膜破裂时间(BUT)、泪腺分泌(SIT)]变化,统计两组患者的并发症及复发率。结果 观察组总有效率为95.83%,高于对照组(81.25%,P<0.05);治疗后,两组的角膜水平曲度、角膜垂直曲度、视力均升高,且观察组高于对照组,而角膜散光度降低,观察组低于对照组(P<0.05);治疗后,两组的BUT、SIT均升高,且观察组的BUT、SIT高于对照组(P<0.05);观察组并发症发生率为4.17%、复发率2.08%均分别低于对照组(16.67%、14.58%,P<0.05)。结论 改良式翼状胬肉切除+CAT治疗翼状胬肉的效果显著,可有效提升视觉质量,改善泪膜功能,且并发症发生率、复发率低,可在临床推广运用。

Objective To explore the short-term and distant efficacy of modified pterygium excision combined with autologous corneal limbal stem cell transplantation(CAT)for the treatment of pterygium.Methods 96 patients with pterygium who were treated in our hospital from June 2021 to January 2023.They were divided into two groups,48 cases in each group.Patient in the control group received modified pterygium excision treatment,while in the observation group received modified pterygium excision+CAT treatment.The treatment effects of the two groups were compared,and changes in visual quality,tear film function[tear film rupture time(BUT),and tear gland secretion(SIT)]before and after treatment were recorded,and the complications and recurrence rates of the two groups of patients were statistically analyzed.Results The total effective rate of the observation group was 95.83%,which was higher than that of the control group(81.25%,P<0.05).After treatment,the horizontal curvature,vertical curvature,and visual acuity of the cornea in both groups increased,and the observation group was higher than the control group,while the corneal astigmatism decreased,and the observation group was lower than the control group(P<0.05).After treatment,the BUT and SIT of both groups increased,and the BUT and SIT of the observation group were higher than those of the control group(P<0.05).The incidence of complications and recurrence rate in the observation group were 4.17%and 2.08%,respectively,lower than those in the control group(16.67%and 14.58%,P<0.05).Conclusion The improved pterygium resection combined with CAT has a significant effect on the treatment of pterygium,which can effectively improve visual quality,tear film function,and have a low incidence of complications and recurrence,which was widely used in clinical practice.
