2019—2020年喀什地区部分医疗机构梅毒病例报告准确性及影响因素分析

Verification and Analysis of Accuracy of Syphilis Case Reports in Some Medical Institutions of Kashgar from 2019 to 2020

目的:分析喀什地区部分医疗机构梅毒病例报告准确性及影响因素。方法:选择2019年1月—2021年1月喀什地区部分医疗机构报告的梅毒病例信息86例。依据《梅毒、淋病、生殖器疱疹、生殖道沙眼衣原体感染诊疗指南(2014年)》及“性病病例现场核查方案”的标准,核查病例报告准确性,并采用logistic回归分析其影响因素。结果:本次核查分期准确79例(91.86%),实验室诊断准确79例(100.00%),疑似病例证实为梅毒1例(14.29%),总准确例数80例(93.02%)。医疗机构类型、行政级别、报告科室、临床医生的文化程度、职称、近3年参加过性病培训、临床医生分类比较,差异有统计学意义(P<0.05)。其中,行政级别、报告科室(非皮肤科)、临床医生的文化程度(专科)、职称(中级及以下)、近3年参加过性病培训(否)、临床医生分类(非专科)是影响梅毒病例报告准确性的独立危险因素(P<0.05)。结论:喀什地区部分医疗机构梅毒病例报告准确性较高,影响其报告准确性既有医疗机构的因素,也有医生自身的因素。

OBJECTIVE To analyze the accuracy verification of syphilis case reports and its influencing factors in some medical institution of kashgar.METHODS 86 syphilis cases reported by hospital institutions from January 2019 to January 2021 were select⁃ed.According to the Guidelines for Diagnosis and Treatment of Syphilis,Gonorrhea,Genital herpes,and Chlamydia Trachomatis In⁃fection of the Reproductive Tract(2014)and the Site Verification Plan for STD Cases,the survey checked the information of 86 re⁃ported syphilis cases,and questionnaire was used to investigate the type of medical institutions and doctors.Exploring the accuracy verification of syphilis case reports and using logistic regression analysis to analyze influencing factors.RESULTS In this verification,79 cases(91.86%)were accurately staged,79 cases(100.00%)were accurately diagnosed in the laboratory,and 1 suspected case was confirmed as syphilis(14.29%).The total number of accurate cases was 80 cases(93.02%).There was a statistically significant difference(P<0.05)in the type of medical institution,administrative level,reporting department,education level and professional ti⁃tle of clinical doctors,as well as in the past 3 years of sexually transmitted disease training.Among them,administrative level,report⁃ing department(non dermatology),education level of clinical doctors(specialized),professional title(intermediate or below),and par⁃ticipation in sexually transmitted disease training in the past 3 years were statistically significant(P<0.05).Clinician classification(non specialized)is an independent risk factor that affects the accuracy of syphilis case reporting(P<0.05).CONCLUSION The accu⁃racy of syphilis case reporting is relatively high,which is influenced by both medical institutions and doctors themselves.
