穴位按摩联合握拳运动对预防肿瘤患者PICC相关性血栓的效果研究

Effects of acupoint massage combined with fist movement on prevention of PICC-associated thrombus in tumor patients

目的探讨穴位按摩联合握拳运动对预防肿瘤患者PICC相关性血栓的效果.方法选取2021年7月-2022年7月在肿瘤内科住院并首次置入PICC导管的肿瘤患者88例,采用随机数字表法将其分为对照组和观察组,各44例.对照组在PICC常规护理基础上行置管侧上肢的握拳运动,观察组在对照组基础上增加穴位按摩.比较2组穿刺侧上肢腋静脉血流速度及静脉血栓发生率.结果干预后,观察组穿刺侧上肢腋静脉平均血流速度及最大血流速度均大于对照组(t=2.086,P=0.040;t=2.000,P=0.048),静脉血栓发生率低于对照组(χ^(2)=4.666,P=0.031).结论将穴位按摩联合握拳运动应用于肿瘤PICC置管患者,可以有效加快置管侧静脉血流速度,降低导管相关性血栓发生率,值得临床推广应用.

Objective To explore the effect of acupoint massage combined with fist movement on preventing peripherally inserted central catheter(PICC)-associated thrombosis in cancer patients.Methods A total of 88 tumor patients with PICC catheter implantation for the first time in the department of oncology between July 2021 and July 2022 were selected.They were divided into an observation group and a control group,each of 44,according to a random number table.Both groups received routine nursing for PICC and fist movement,while the observation group additionally received acupoint massage combined with clenched fist exercise.The blood flow velocity of the axillary vein on the affected side and the incidence of venous thrombosis were compared between the two groups.Results After the intervention,the average and maximum blood flow velocity of axillary vein of the observation group were significantly quicker than that of the control group(t=2.086,P=0.040;t=2.000,P=0.048),while the incidence of venous thrombosis was significantly lower than the latter(χ^(2)=4.666,P=0.031).Conclusion Acupoint massage combined with fist clenching exercise can improve blood circulation in patients with PICC and prevent PICC-related venous thrombosis in the upper limb.Therefore,it is worthy of application in the clinical practice.
