腹腔镜肝癌根治术与开腹手术治疗肝癌的效果比较

Comparison of therapeutic effect of laparoscopic radical hepatectomy and open surgery on liver cancer

目的比较腹腔镜肝癌根治术与开腹手术治疗肝癌的效果。方法抽取2020年4月至2022年4月郑州大学第一附属医院收治的肝癌患者148例,按照手术方案分为对照组和观察组,每组74例。对照组给予开腹手术治疗,观察组给予腹腔镜根治术治疗。比较两组手术指标、手术前后的炎性因子及免疫功能指标。结果观察组手术出血量少于对照组,切口长度、绝对卧床时间、禁食时间、住院时间短于对照组(P<0.05)。术前,两组炎性因子、免疫功能指标比较差异未见统计学意义(P>0.05);术后3 d,观察组C-反应蛋白、CD8^(+)低于对照组(t=17.39、9.40,P均<0.05),白细胞介素-2、CD3^(+)、CD4^(+)高于对照组(t=4.01、9.67、9.72,P均<0.05)。结论与开腹手术比较,腹腔镜根治术治疗肝癌可缩短术后康复时间,减少出血量,切口更小,术后炎症反应更轻,且能更好地保护机体免疫功能。

Objective To compare the therapeutic effect of laparoscopic radical hepatectomy and open surgery on liver cancer.Methods A total of 148 patients with liver cancer admitted to the First Affiliated Hospital of Zhengzhou University from April 2020 to April 2022 were selected,and the patients were divided into control group and observation group according to the operation plan,with 74 cases in each group.The control group was treated by open surgery,while the observation group was treated by laparoscopic radical hepatectomy.The surgical indexes,preoperative and postoperative inflammatory factors and immune function indexes were compared between the two groups.Results The surgical blood loss in the observation group was less than that in the control group,moreover,the incision length,absolute bed time,fasting time,and hospital stay in the observation group were shorter than those in the control group(P<0.05).Before operation,there was no significant difference in inflammatory factors and immune function indexes between the two groups(P>0.05).Three days after operation,the C-reactive protein and cluster of differentiation 8^(+)in the observation group were lower than those in the control group(t=17.39,9.40;all P<0.05),the levels of interleukin-2,cluster of differentiation 3^(+)cluster of differentiation 4^(+)in the observation group were higher than those in the control group(t=4.01,9.67,9.72;all P<0.05).Conclusions Compared with open surgery,laparoscopic radical hepatectomy for liver cancer can shorten postoperative recovery time,and reduce blood loss,with smaller incision and less postoperative inflammatory reaction,which can better protect the immune function of the body.
