宫腔镜刨削系统治疗黏膜下子宫肌瘤的效果研究

Effect of hysteroscopic intergrated Bigatti shaver in the treatment of submucosal uterine fibroids

目的分析宫腔镜刨削系统治疗黏膜下子宫肌瘤的临床价值。方法抽取2019年4月至2022年4月安阳市人民医院收治的黏膜下子宫肌瘤患者96例,按随机数字表法分为对照组与研究组,每组48例。对照组采用宫腔镜双极电切术,研究组采用宫腔镜刨削系统切除术。比较两组围术期情况(手术时间、术中出血量、术后阴道出血时间)、术后恢复优良率、术后1个月子宫内膜厚度、术前及术后30 min生化指标(血钾、血钠)、并发症发生率。结果研究组术中出血量少于对照组,手术时间和术后阴道出血时间短于对照组(P<0.05)。研究组术后恢复优良率(95.83%,46/48)高于对照组(81.25%,39/48),差异有统计学意义(χ^(2)=5.03,P=0.025)。研究组术后1个月子宫内膜厚度大于对照组(P<0.05)。术后30 min,两组血钾、血钠水平与术前比较差异未见统计学意义,且两组间比较差异未见统计学意义(P>0.05)。研究组并发症发生率(4.17%,2/48)低于对照组(16.67%,8/48),差异有统计学意义(P<0.05)。结论黏膜下子宫肌瘤患者采用宫腔镜刨削系统切除术治疗,可减少术中出血量,缩短术后阴道出血时间,加快术后机体恢复,并可降低并发症发生率,提高手术安全性。

Objective To analyze the value of hysteroscopic intergrated Bigatti shaver in the treatment of submucosal uterine fibroids.Methods A total of 96 patients with submucosal uterine fibroids treated in the People’s Hospital of Anyang City from April 2019 to April 2022 were selected,and they were divided into control group and study group according to random number table method,with 48 cases in each group.The control group was treated by hysteroscopic bipolar electrocision,and the study group was treated by hysteroscopic intergrated Bigatti shaver.The perioperative conditions(operation time,intraoperative bleeding volume,postoperative vaginal bleeding time),excellent and good rate of postoperative recovery,1-month postoperative endometrial thickness,preoperative and 1-month postoperative biochemical indexes(blood potassium,blood sodium),and the incidence of complications were compared between the two groups.Results The intraoperative bleeding volume in the study group was less than that in the control group,while the operation time and postoperative vaginal bleeding time were shorter than those in the control group(P<0.05).The excellent and good rate of postoperative recovery in the study group(95.83%,46/48)was higher than that in the control group(81.25%,39/48),and the difference was significant(χ^(2)=5.03,P<0.05).The 1-month postoperative endometrial thickness of the study group was thicker than that of the control group(P<0.05).There were no significant difference in preoperative and postoperative levels of serum potassium and sodium in the two groups,and there was no significant difference in levels of serum potassium and sodium between the two groups 30 min after operation(P>0.05).The incidence of complications in the study group(4.17%,2/48)was lower than that in the control group(16.67%,8/48),P<0.05.Conclusions Hysteroscopic intergrated Bigatti shaver in the treatment of submucosal uterine fibroids can reduce intraoperative blood loss,shorten postoperative vaginal bleeding time,speed up postoperative recovery,reduce the incidence of complications,and improve surgical safety.
