特异性促炎症消退介质在骨病治疗中的研究进展

摘要 骨骼作为一个独特的器官,不断地进行骨重塑,维持骨骼的生理稳态,为身体提供支持。在各种急慢性骨损伤中,初期往往伴随着炎症的出现。炎症是组织对有害刺激的保护性反应,炎症的产生在一定程度上是有益的,可以将免疫细胞招募到组织损伤或感染的部位,引导有害刺激的移除和愈合过程的启动。如果炎症不能及时消退,则会导致局部或全身性炎症慢性化,破坏骨稳态,从而引起和加重骨骼疾病。已有研究将全身性、慢性、低水平炎症作为骨质疏松症(osteoporosis,OP)、类风湿关节炎(rheumatoid arthritis,RA)和牙周炎发病机制的一部分。特异性促炎症消退介质(specialized pro-resolving mediators,SPMs)是一类多不饱和脂肪酸(polyunsaturated fatty acids,PUFAs)的代谢产物,研究发现SPMs在促进炎症消退和组织再生方面疗效显著。在此基础上也有研究证实SPMs对骨细胞有直接作用,能抑制骨破坏,促进骨形成。本文就SPMs对骨病的治疗作用,从促进炎症消退、防止骨破坏和促进骨形成的不同途径作一综述,以期为探讨利用SPMs治疗骨病的研究提供思路。 As a unique organ,bone can continuously remodel and maintain physiological homeostasis to support the body.In various acute and chronic bone injuries,inflammation is often accompanied at the initial stage,and inflammation is the protective response of tissues to harmful stimuli.The production of inflammation is beneficial to a certain extent.It can recruit immune cells to the site of tissue damage or infection,guide the removal of harmful stimuli,and initiate the healing process.If the inflammation does not resolve in time,it will lead to local or systemic inflammation chronicization,and disrupt bone homeostasis,causing and aggravating bone diseases.Systemic,chronic,and low-grade inflammation has been studied as part of the pathogenesis of osteoporosis,rheumatoid arthritis,and periodontitis.Specialized,pro-resolving mediators(SPMs)are a class of polyunsaturated fatty acids(PUFAs)metabolites.SPMs have been found to be effective in promoting chronic inflammation resolution and tissue regeneration.Studies have confirmed that SPMs have a direct effect on bone cells,can inhibit bone destruction,and promote bone formation.This article reviews the therapeutic effect of SPMs on chronic inflammation in bone disease,from promoting inflammation resolution to preventing bone destruction and promoting bone formation.In order to provide ideas for the study of the use of SPMs in the treatment of bone diseases.
