行动管理结合吞咽训练在老年脑卒中后吞咽障碍患者中的应用

Application of Action Management Combined with Swallowing Training in Elderly Patients with Dysphagiaafter Stroke

目的:探讨行动管理结合吞咽训练在老年脑卒中后吞咽障碍患者中的应用效果。方法:选取2020年5月1日~2022年9月1日收治的90例老年脑卒中后吞咽障碍患者作为研究对象,按照随机数字表法将患者分为对照组和研究组各45例,对照组实施常规吞咽训练,研究组在常规基础上结合行动管理;比较两组训练依从性,干预前后吞咽功能[采用中文版吞咽功能评估量表(GUSS)]、希望水平[采用Herth希望量表(HHI)]、生活质量[采用吞咽障碍生活质量量表(EDQOL)]。结果:研究组训练依从性高于对照组(P<0.01);干预后,两组GUSS、EDQOL评分低于干预前(P<0.05),且研究组低于对照组(P<0.01);干预后,两组HHI水平高于干预前(P<0.05),且研究组高于对照组(P<0.01)。结论:对老年脑卒中后吞咽障碍患者实施行动管理结合吞咽训练干预,可有效恢复患者吞咽功能,提高其希望水平和生活质量。

Objective:To explore the application effect of action management combined with swallowing training with dysphagia after stroke.Methods:A total of 90 elderly patients with dysphagia after stroke admitted from May 1,2020 to September 1,2022 were selected as the study objects and were by the random number table method divided into the control group and the study group with 45 cases in each group.The control group received routine swallowing training intervention while the study group was combined with action management intervention on this basis.The training compliance,and swallowing function by Chinese version of Gugging Swallowing Screen(GUSS),hope level by Herth Hope Inventory(HHI)and quality of life by Eating Disorders Quality of Life Scale(EDQOL)before and after intervention were compared between the two groups.Results:The training compliance of patients in the study group was significantly higher than that in the control group(P<0.01).After 2 months of intervention,the scores of GUSS and EDQOL in the two groups were significantly lower than those before intervention(P<0.05),and those in the study group were significantly lower than those in the control group(P<0.01).The HHI scores in the two groups were significantly higher than those before intervention(P<0.05),and those in the study group were significantly higher than those in the control group(P<0.01).Conclusion:Action management combined with swallowing training intervention for elderly patients with dysphagia after stroke can effectively restore the swallowing function,and improve the hope level and quality of life.
