囊袋张力环植入联合超声乳化术治疗高度近视并发白内障的效果

Effect of capsular tension ring implantation combined with phacoemulsification on high myopia complicated withcataract

目的 探讨囊袋张力环植入(CTR)联合超声乳化术治疗高度近视并发白内障的效果。方法 回顾性分析2020年1月至2022年11月平顶山市第一人民医院收治的78例(156眼)高度近视并发白内障患者的临床资料,所有患者均接受超声乳化白内障吸出术,将接受人工晶状体和CTR植入的40例患者纳入观察组,将采用人工晶状体植入的38例患者纳入对照组,比较两组术后视力水平、前房深度、人工晶状体位置、屈光度以及术后并发症发生情况。结果 两组术后1周、术后1、3、6个月时裸眼视力(UCVA)、最佳矫正视力(BCVA)logMAR水平比较,差异未见统计学意义(P>0.05)。两组术后1周、术后1个月前房深度比较,差异未见统计学意义(P>0.05)。观察组术后3、6个月时前房深度大于对照组,差异有统计学意义(P<0.05)。两组术后1周时有效人工晶体位置(ELP)均值比较,差异未见统计学意义(P>0.05)。观察组术后1、3、6个月时ELP均值以及总体ELP均方根值小于对照组(P<0.05)。术后3、6个月时观察组屈光度飘移小于对照组(P<0.05)。观察组术后并发症发生率低于对照组,差异均有统计学意义(P<0.05)。结论 在超声乳化术与人工晶状体植入的基础上联合使用CTR有利于稳定人工晶状体位置和前房深度,减少屈光度飘移,降低术后并发症发生风险。

Objective To investigate the effect of capsular tension ring implantation(CTR)combined with phacoemulsification on high myopia with cataract.Methods A retrospective analysis was conducted on the clinical data of 78 patients(156 eyes)with high myopia and cataract admitted to the First People's Hospital of Pingdingshan City from January 2020 to November 2022.All patients underwent phacoemulsification cataract extraction.Forty patients who received intraocular lens and CTR implantation were included in the observation group,and 38 patients who received intraocular lens implantation were included in the control group.The postoperative visual acuity,anterior chamber depth,the intraocular lens position,dioptre and postoperative complications of the artificial lens were compared.Results There was no significant difference in the levels of uncorrected visual acuity(UCVA)and best corrected visual acuity(BCVA)logMAR between the two groups at 1 week,1 month,3 months,and 6 months after surgery(P>0.05).There was no significant difference in anterior chamber depth between the two groups at 1 week and 1 month after surgery(P>0.05).The depth of the anterior chamber in the observation group was greater than that in the control group at 3 and 6 months after surgery,with significant differences(P<0.05).There was no significant difference between the two groups in the mean effective intraocular lens position(ELP)1 week after surgery(P>0.05).The mean ELP and overall ELPRMS values of the observation group were lower than those of the control group at 1 month,3 months,and 6 months after surgery(P<0.05).The refractive deviation of the observation group was lower than that of the control group at 3 months,and 6 months after surgery(P<0.05).The incidence of postoperative complications of the observation group was lower than that of the control group,and the difference was significant(P<0.05).ConclusionsThe combination of phacoemulsification and intraocular lens implantation with CTR is beneficial for stabilizing the position and anterior chamber depth of the intraocular lens,reducing refractive deviation,and reducing the risk of postoperative complications.
