微创颌骨手术联合正畸治疗改善上颌前突的临床研究 被引量:1

Clinical study of minimally invasive jaw surgery combined with orthodontic treatment to relieve maxillary protrusion

摘要 目的评估微创颌骨手术联合正畸治疗改善上颌前突的临床疗效。方法本研究为回顾性研究,选取40例上颌前突女性患者,其中20例采用微创颌骨手术联合正畸治疗,20例采用单纯拔牙正畸治疗,对其治疗前后的头颅定位侧位片测量,对数据进行统计分析,比较两种治疗方式的术后效果。结果微创颌骨手术联合正畸治疗前后大部分矢状向、垂直向测量指标差异均有统计学意义。研究表明,上中切牙切缘点微创颌骨手术联合正畸治疗后内收4.17(2.80)mm,单纯拔牙正畸治疗后内收2.38(2.93)mm,P<0.001;上中切牙切缘点到横坐标轴的垂直距离微创颌骨手术联合正畸治疗后减小1.16(2.62)mm,单纯拔牙正畸治疗后减小1.49(4.98)mm,P<0.001。结论相比于单纯拔牙正畸治疗,微创颌骨手术联合正畸治疗后上前牙内收更多,实现压入性内收,更有利于侧貌改善。 Objective To evaluate the clinical effect of minimally invasive jaw surgery combined with orthodontic treatment to improve maxillary protrusion.Methods This study is a retrospective study.40 female patients with maxillary protrusion were selected,of which 20 were treated with minimally invasive jaw surgery combined with orthodontic treatment,and 20 were treated with simple tooth extraction orthodontic treatment.The data were measured before and after treatment,and the data were statistically analyzed to compare the postoperative effects of the two treatment methods.Results Most of the sagittal and vertical measurement indexes before and after minimally invasive jaw surgery combined with orthodontic treatment were statistically significant.The comparative study showed that the upper central incisor point retracted by 4.17(2.80)mm after minimally invasive jaw surgery combined with orthodontic treatment,and 2.38(2.93)mm after orthodontic treatment alone,P<0.001;The vertical distance from the upper central incisor point to the abscissa axis decreased by 1.16(2.62)mm after minimally invasive jaw surgery combined with orthodontic treatment and 1.49(4.98)mm after orthodontic treatment alone,P<0.001.Conclusions Compared with the orthodontic treatment of tooth extraction alone,minimally invasive jaw surgery combined with orthodontic treatment can achieve more anterior maxillary adduction and achieve compressive adduction,which is more conducive to the improvement of profile.
