彩色多普勒超声在乳腺良恶性肿瘤诊断中的应用价值

摘要 目的探究乳腺良恶性肿瘤诊断中彩色多普勒超声的应用价值。方法选取2018年3月至2020年12月本院超声科收治的114例乳腺良恶性肿瘤患者作为研究对象,患者均行彩色多普勒超声及病理检查,分析其肿块边界、形态、内部回声、肿块血流等情况,以病理检查结果为金标准,比较两种检查方式在临床中的应用价值。结果病理诊断结果显示,良性乳腺肿瘤86例,其中乳头状瘤25例,纤维腺瘤29例,结节增生32例;检出恶性乳腺肿瘤28例,其中浸润性导管癌24例,黏液癌2例,髓样癌2例。诊断结果显示,共检出良性乳腺肿瘤83例,占比96.51%,恶性乳腺肿瘤检出25例,占比89.29%,两种检查方式检出率比较差异无统计学意义。恶性乳腺肿瘤阻力指数(RI)大于良性乳腺肿瘤,最高血流速度(V_(max))高于良性乳腺肿瘤,差异有统计学意义(P<0.05)。良性乳腺肿瘤0级、Ⅰ级占比均高于恶性乳腺患者,Ⅱ级、Ⅲ级占比均低于恶性乳腺患者,差异有统计学意义(P<0.05)。恶性乳腺肿瘤患者边界毛刺状、形态不规则、包膜不完整、内部钙化占比均高于良性乳腺肿瘤患者,差异有统计学意义(P<0.05)。结论彩色多普勒超声具有性价比高、操作简单、无创伤性、可重复等特征,可广泛用于乳腺良恶性肿瘤的临床诊断中,诊断价值高,值得临床推广应用。 Objective To explore the application value of color Doppler ultrasound in the diagnosis of benign and malignant breast tumors.Methods 114 patients with benign and malignant breast tumors who were treated in the ultrasound department of our hospital from March 2018 to December 2020 were selected as the study subjects,all patients underwent color Doppler ultrasonography and pathological examination,the tumor boundary,morphology,internal echo and blood flow of the tumors were analyzed,and the clinical application value of the two methods was compared with the pathological examination results as the gold standard.Results Pathological diagnosis showed that there were 86 benign breast tumors,including 25 papillomas,29 fibroadenomas and 32 nodular hyperplasia.Malignant breast tumors were detected in 28 cases,including infiltrating ductal carcinoma in 24 cases,mucous carcinoma in 2 cases and medullary carcinoma in 2 cases.The diagnostic results showed that 83 cases of benign breast tumors were detected,accounting for 96.51%,and 25 cases of malignant breast tumors were detected,accounting for 89.29%.There was no statistical significance in the detection rate between the two methods.The resistance index(RI)and_(max)imum velocity(V_(max))of malignant breast tumors were higher than those of benign breast tumors,and the differences were statistically significant(P<0.05).The proportion of benign breast tumors of grade 0 and gradeⅠwas higher than that of malignant breast patients,and the proportion of gradeⅡand gradeⅢwas lower than that of malignant breast patients,the difference was statistically significant(P<0.05).The percentages of burr,irregular shape,incomplete envelope and internal calcification in patients with malignant breast tumor were higher than those with benign breast tumor,and the differences were statistically significant(P<0.05).Conclusion Color ultrasound has the characteristics of high cost performance,simple operation,non-invasive,repeatable,etc.It can be widely used in the clinical diagnosis of benign and malignant breast tumors,and has high diagnostic value,worthy of clinical promotion and application.
