MRI联合多模态超声对早期乳腺癌的诊断价值分析

摘要 目的探究MRI联合多模态超声对早期乳腺癌的诊断价值。方法选取2018年4月至2020年12月于本院影像科检查的80例乳腺病变患者作为研究对象,均行病理检查、多模态超声、MRI检查,以病理检查为金标准,比较不同检查方法的恶性病变检出率、漏诊率、误诊率、显示征象及其对乳腺癌的诊断价值。结果经病理检查,80例乳腺病变患者中包括恶性病变70例,良性病变10例。MRI检出恶性病变47例,良性病变33例;多模态超声检出恶性病变53例,良性病变27例;MRI联合多模态超声检测出恶性病变66例,良性病变14例。MRI、多模态超声检查与MRI联合多模态超声检查的特异度、阳性预测值、阴性预测值比较差异无统计学意义;MRI、多模态超声检查与MRI联合多模态超声检查的准确率比较差异有统计学意义(P<0.05)。MRI与多模态超声检查的灵敏度、特异度、阳性预测值、阴性预测值比较差异无统计学意义;MRI联合多模态超声检查的灵敏度、准确率高于MRI、多模态超声,差异有统计学意义(P<0.05)。MMRI、多模态超声检查与MRI联合多模态超声检查检出率、漏诊率比较差异有统计学意义(P<0.05);MRI、多模态超声检查与MRI联合多模态超声检查误诊率比较差异无统计学意义。MRI与多模态超声检查的检出率、漏诊率、误诊率比较差异无统计学意义;MRI联合多模态超声检查检出率高于MRI、多模态超声,MRI联合多模态超声检查漏诊率低于MRI、多模态超声,差异有统计学意义(P<0.05)。MRI、多模态超声检查与MRI联合多模态超声检查肿块、钙化、多形性、不均质性及局部淋巴结转移的检出率比较差异有统计学意义(P<0.05);MRI与多模态超声检查肿块、钙化、多形性、不均质性及局部淋巴结转移的检出率比较差异无统计学意义;MRI联合多模态超声检查肿块、钙化、多形性、不均质性及局部淋巴结转移的检出率高于MRI、多模态超声检出率,差异有统计学意义(P<0.05)。结论MRI联合多模态超声检查可提高早期乳腺癌的检出率、灵敏度、准确率,诊断价值较高。 Objective To explore the diagnostic value of MRI combined with multimodal ultrasound in early breast cancer.Methods A total of 80 patients with breast lesions who were examined in the imaging department of our hospital from April 2018 to December 2020 were selected as the study subjects,and all of them underwent pathological examination,multimodal ultrasound and MRI.Pathological examination was taken as the gold standard,and all the examination results were recorded,the detection rate of malignant lesions,the rate of missed diagnosis,the rate of misdiagnosis,the display signs and the diagnostic value of different examination methods were compared.Results After pathological examination,80 cases of breast lesions included 70 cases of malignant lesions and 10 cases of benign lesions.MRI detected 47 cases of malignant lesions and 33 cases of benign lesions.There were 53 malignant lesions and 27 benign lesions detected by multimodal ultrasound.MRI combined with multimodal ultrasound detected 66 cases of malignant lesions and 14 cases of benign lesions.There was no significant difference in the specificity,positive predictive value and negative predictive value of MRI,multimodal ultrasound and MRI combined multimodal ultrasound.The accuracy of MRI,multimodal ultrasound and MRI combined multimodal ultrasound was significantly different(P<0.05).There was no significant difference in sensitivity,specificity,positive predictive value and negative predictive value between MRI and multimodal ultrasound.The sensitivity and accuracy of MRI combined with multimodal ultrasound were higher than those of MRI and multimodal ultrasound,and the difference was statistically significant(P<0.05).There were significant differences in the detection rate and missed diagnosis rate between MMRI,multimodal ultrasound and MRI combined multimodal ultrasound(P<0.05).There was no significant difference in the misdiagnosis rate between MRI,multimodal ultrasound and MRI combined multimodal ultrasound.There was no significant difference in the detection rate,missed diagnosis rate and misdiagnosis rate between MRI and multimodal ultrasound.The detection rate of MRI combined with multimodal ultrasound was higher than that of MRI and multimodal ultrasound,and the missed diagnosis rate of MRI combined with multimodal ultrasound was lower than that of MRI and multimodal ultrasound,the difference was statistically significant(P<0.05).There were significant differences in the detection rates of mass,calcification,polymorphism,heterogeneity and local lymph node metastasis between MRI,multimodal ultrasound and MRI combined multimodal ultrasound(P<0.05).There was no significant difference in the detection rates of mass,calcification,polymorphism,heterogeneity and local lymph node metastasis between MRI and multimodal ultrasound.The detection rates of mass,calcification,polymorphism,heterogeneity and local lymph node metastasis detected by MRI combined with multimodal ultrasound were higher than those by MRI and multimodal ultrasound,and the difference was statistically significant(P<0.05).Conclusion MRI combined with multimodal ultrasound can improve the detection rate,sensitivity and accuracy of early breast cancer,and has high diagnostic value.
