阴道超声检查在宫角妊娠与输卵管间质部妊娠鉴别诊断中的价值分析

The Value of Transvaginal Ultrasonography in the Differential Diagnosis of Cornual Pregnancy and Tubal Interstitial Pregnancy

目的:探讨阴道超声检查在宫角妊娠与输卵管间质部妊娠的鉴别诊断中的价值.方法:选取2019年3月至2023年1月在本院就诊的宫角妊娠和输卵管间质部妊娠患者72例为研究对象.根据术后病理学检查结果,将其分为宫角妊娠组(n=38)和输卵管间质部妊娠组(n=34).以术后病理学检查结果为"金标准",分析阴道超声检查在宫角妊娠与输卵管间质部妊娠的鉴别诊断中的价值,并分析两类妊娠的超声影像学特征.结果:阴道超声检查诊断的总准确率、宫角妊娠和输卵管间质部妊娠的准确率分别为88.88%(64/72)、89.47%(34/38)、88.23%(30/34).宫角妊娠组与输卵管间质部妊娠组在宫角膨隆、间质特征的阴道超声影像特征比较,差异无统计学意义(P>0.05);而在孕囊外凸、周边肌层完整、子宫内膜包绕或紧邻、宫腔完整等超声影像特征比较,差异有统计学意义(P<0.05).结论:阴道超声检查在宫角妊娠与输卵管间质部妊娠中,均具有较高的诊断准确性,且二者的超声图像特征有差异,能够作为二者鉴别的重要依据,具有一定的临床价值.

Objective:To explore the differential diagnostic value of transvaginal ultrasonography between cornual pregnancy and tubal interstitial pregnancy.Methods:Seventy-two patients with cornual pregnancy and tubal interstitial pregnancy admitted in our hospital from March 2019 to January 2023 were selected in this study.According to the results of postoperative pathological examination,they were divided into cornual pregnancy group(n=38)and tubal interstitial pregnancy group(n=34).Based on the results of postoperative pathological examination as the"gold standard",the value of vaginal ultrasound in the dfferential diagnosis of cornual pregnancy and tubal interstitial pregnancy was analyzed,and the ultrasonic imaging characteristics of the two types of pregnancy were analyzed.Results:The total accuracy of vaginal ultrasound diagnosis was 88.88%(64/72),with 89.47%(34/38)accuracy for cornual pregnancy and 88.23%(30/34)accuracy for tubal interstitial pregnancy respectively.There were no statistically significant difference in the vaginal ultrasound imaging features of uterine horn distention and interstitial features between two groups(P>0.05),while there were statistically significant differences in ultrasound imaging features in protrusion of pregnancy sac,complete peripheral muscle layer,enclosed or adjacent endometrium and complete uterine cavity between two groups(P<0.05).Conclusions:Transvaginal ultrasound examination has high diagnostic accuracy in cornual pregnancy and tubal interstitial pregnancy.There are differences in the ultrasound image characteristics between the two,which can serve as an important basis for distinguishing the two and have certain clinical value.
