细胞DNA单倍体定量联合人乳头瘤病毒液基薄层细胞学检查在超早期宫颈癌筛查中的效能 被引量:1

摘要 目的探讨细胞DNA单倍体定量联合人乳头瘤病毒(HPV)、液基薄层细胞学(TCT)检查在超早期宫颈癌筛查中的效果。方法选择2020年4月至2022年1月疑似超早期宫颈癌患者92例为对象,所有患者均完成细胞DNA单倍体、HPV及TCT检查,以病理组织检查结果作为“金标准”,分析细胞DNA单倍体、HPV、TCT检查及三者联合检查在超早期宫颈癌筛查中的检出率,计算并分析不同筛查方法在超早期宫颈癌中的筛查效能(灵敏度与特异度)。结果92例疑似超早期宫颈癌患者经病理检查最终确诊64例,确诊率为70%。细胞DNA单倍体定量检测、HPV联合TCT检查在超早期宫颈癌中检查阳性62例,检查准确度为93.48%(86/92)、灵敏度为93.75%(60/64)、特异度为92.86%(26/28),表明细胞DNA单倍体、HPV联合TCT检查在超早期宫颈癌患者中的诊断灵敏度与特异度高于单一方法筛查,差异有统计学意义(P<0.05)。结论细胞DNA单倍体定量、HPV、TCT检查用于超早期宫颈癌筛查中均具有较高的检出率,三者方法联合筛查有助于提升筛查效能。 Objective To explore the effect and efficiency of cell DNA ploidy quantitative combined with human papillomavirus(HPV)and liquid-based thin layer cytology(TCT)in the screening of ultra-early cervical cancer.Methods Ninety-two patients with suspected extra-early cervical cancer from April 2020 to January 2022 were selected as the subjects.All patients were routinely examined for cell DNA haploid,HPV and TCT.The results of the pathological examination were used as the"gold standard"to analyze the detection rate of cell DNA haploid,HPV,TCT and their combined examination in the screening of extra-early cervical cancer.The screening efficacy(sensitivity and specificity)of different screening methods in ultra-early cervical cancer were calculated and analyzed.Results Sixty-four of 92 patients with suspected extra-early cervical cancer were finally diagnosed by pathological examination,with a diagnosis rate of 70%.Cell DNA haploid,HPV and TCT were positive in 62 cases in the screening of ultra-early cervical cancer,with an accuracy of 93.48%(86/92),sensitivity of 93.75%(60/64)and specificity of 92.86%(26/28).The calculation results showed that the sensitivity and specificity of cell DNA haploid,HPV combined with TCT in the diagnosis of ultra-early cervical cancer patients were higher than those of single-method screening,and the differences were statistically significant(P<0.05).Conclusion Cell DNA haploid,HPV and TCT have high detection rates in the screening of ultra-early cervical cancer,and the combined screening of the three methods can help improve the screening efficiency.
