腹透液导致腹膜透析患者湿困脾土的理论探讨

Theoretical Study on Peritoneal Dialysis Patients with Wet Spleen Soil Caused by Peritoneal Dialysis

慢性肾衰竭属于慢性肾脏病的终末期阶段,只用药物无法控制病情进展,患者不得不选择肾脏替代治疗。近年来,随着患病率不断提高,选择腹膜透析的患者也越来越多。腹膜透析虽然给患者减轻了痛苦,但长期腹透带来的并发症也不可忽略。由于腹透患者腹腔内长期留置腹透液,日久形成湿邪,影响脾胃功能,造成水湿困脾的局面。腹膜透析阶段的病机与慢性肾衰相同,均为脾肾两虚,脾虚不运,影响水液代谢,水湿积聚体内,再加腹透液留置体内产生的外来湿邪,湿困脾土,共同影响脾脏。文章从腹膜透析患者自身体质以及脾胃与水湿之间的关系探讨了湿邪产生的过程、对脾胃的影响以及相关症状,证明了腹透患者腹透液长期留腹产生湿邪,加重对脾胃的影响,为腹膜透析患者治疗不可忽略脾胃提供理论依据。

Chronic renal failure belongs to the end-stage of chronic kidney disease.Only drugs can not control the progress of the disease.Patients have to choose renal replacement therapy.In recent years,with the increasing prevalence,more and more patients choose peritoneal dialysis.Although peritoneal dialysis has alleviated the pain of patients,the complications caused by long-term peritoneal dialysis can not be ignored.Due to the long-term retention of peritoneal dialysis fluid in the abdominal cavity of peritoneal dialysis patients,dampness evil is formed over time,affecting the function of spleen and stomach,resulting in the situation of water dampness trapping the spleen.The pathogenesis of peritoneal dialysis stage is the same as that of chronic renal failure.Both spleen and kidney deficiency and spleen deficiency do not transport,which affect water and fluid metabolism and water and moisture accumulate in the body.In addition,peritoneal dialysis fluid is retained to produce external dampness evil and dampness trapped in the spleen and soil,which jointly affect the spleen.This paper discusses the process of dampness evil,its influence on the spleen and stomach and related symptoms from the constitution of peritoneal dialysis patients and the relationship between the spleen and stomach and water dampness,and proves that the dampness evil caused by the long-term retention of peritoneal dialysis fluid in peritoneal dialysis patients aggravates the influence on the spleen and stomach,so as to provide a theoretical basis for the treatment of peritoneal dialysis patients who can not ignore the spleen and stomach.
