肝胃不和证诊断标准及应用现状的文献研究

A literature study on the diagnostic criteria and application status of liver-stomach disharmony syndrome

目的分析近5年现代文献中肝胃不和证的诊断标准及其应用现状和规律。方法通过系统整理万方数据、中国知网(CNKI)、维普数据(VIP)等三大数据库2017年—2021年有关肝胃不和证诊断标准的现代文献,将获取的诊断标准使用情况及诊断内容和判定形式等信息整理到Excel表格中,进行频数分析。结果共选出181篇文献,涉及诊断标准59项,使用频次2次以上的19项,包含病名20种,以消化系统疾病居多。出现次数最高的前5位症状分别是胸胁胀满或窜痛(22次)、嗳气(21次)、胃脘胀满或胀痛(21次)、脉弦(20次)、情绪不遂复发或加重(16次),判定形式以主症结合次症为主。结论肝胃不和证诊断标准主要是采用病证结合的模式,多数研究也采用更具有针对性的共识意见等。然而,不同权威机构对于肝胃不和证的分型有不同的看法,需进一步的明确和规范。研究旨在为临床研究提供帮助,共同促进肝胃不和证诊断标准的完善。

Objective To analyze the diagnostic criteria and application status of liver-stomach disharmony syndrome in recent 5 years.Methods In this paper,modern literatures on the diagnostic criteria of disharmony between liver and stomach syndrome from 2017 to 2021 in Wanfang,CNKI and VIP were systematically sorted out,and the information such as the use of diagnostic criteria,diagnosis content and judgment form were sorted into Excel tables for frequency analysis.Results A total of 181 literatures were selected,including 62 diagnostic criteria,19 of which were used more than twice,and 20 of which were diseases of digestive system.The top 5 symptoms with high frequency of occurrence were full distention or pain in the chest and lateral thorax,belching,full distention or pain in the stomach,pulse string,and recurrence or aggravation of emotional dysfunction.The determination form was mainly the combination of primary symptoms and secondary symptoms.Conclusions The diagnosis standard of disharmony between liver and stomach syndrome is mainly based on the combination of disease and syndrome,and more targeted consensus opinion is also adopted in most studies.However,different authorities have different views on the classification of liver-stomach disharmony syndrome,which needs to be further clarified and standardized.The purpose of this study is to provide help for clinical research and jointly promote the improvement of diagnostic criteria of liver and stomach disharmony syndrome.
