益气固摄浓缩丸与奥美沙坦酯治疗慢性肾小球肾炎的效果评价

Effect Evaluation of Yiqi Gushe Nongsuo Pills Combined with Olmesartan Medoxomil in the Treatment of Chronic Glomerulonephritis

目的:明确益气固摄浓缩丸治疗慢性肾小球肾炎(chronic glomerulonephritis,CGN)的效果与安全性。方法:筛选270例九江市中医医院2018年8月—2022年1月收治的CGN患者,根据随机数字表法分为对照组(n=135)及治疗组(n=135)。对照组采用基础治疗,治疗组在对照组基础上施予益气固摄浓缩丸,两组治疗均持续3个月。检测并比较两组治疗前后尿红细胞计数、24 h尿蛋白定量、血清白蛋白(ALB)、尿素氮(BUN)、血肌酐(Scr)、甘油三酯(TG)、总胆固醇(TC)、α1微球蛋白(α1-MG)、视黄醇结合蛋白(RBP)、白介素-4(IL-4)、IL-10、γ干扰素(IFN-γ)、IFN-γ/IL-4、IFN-γ/IL-10,评估两组临床疗效、中医症候积分,详实记录不良反应发生情况。结果:治疗组总有效率(85.93%)高于对照组(71.85%)(P<0.05);治疗后,治疗组尿红细胞计数、24 h尿蛋白定量、BUN、Scr、ALB、TG、TC、α1-MG、RBP均优于对照组(P<0.05);与对照组比较,治疗后治疗组IL-4、IL-10水平均更高,IFN-γ、IFN-γ/IL-4及IFN-γ/IL-10水平均更低(P<0.05);治疗组治疗后中医症候积分、不良反应发生率均低于对照组(P<0.05)。结论:益气固摄浓缩丸通过调节免疫、抑制炎症、调脂、降尿蛋白等途径提高CGN临床疗效,改善疾病预后。

Objective:To determine the efficacy and safety of Yiqi Gushe Nongsuo Pills in the treatment of chronic glomerulonephritis(CGN).Method:A total of 270 patients with CGN who were admitted to Jiujiang Hospital of Traditional Chinese Medicine from August 2018 to January 2022 were screened and divided into the control group(n=135)and the treatment group(n=135)according to random number table method.The control group was given basic treatment,and the treatment group was given Yiqi Gushe Nongsuo Pills on the basis of the control group.The urine red blood cell count,24 h urine protein quantification,blood serum albumin(ALB),blood urea nitrogen(BUN),serum creatinine(Scr),triglyceride(TG),total cholesterol(TC),α1-microglobulin(α1-MG),retinol binding protein(RBP),interleukin-4(IL-4),IL-10,interferon-γ(IFN-γ),IFN-γ/IL-4,and IFN-γ/IL-10 were detected and compared before and after treatment in the two groups.The clinical effect,TCM symptom scores were evaluated in the two groups,and the incidence of adverse reactions and recurrence rate were recorded in detail.Result:The total effective rate in the treatment group(85.93%)was higher than that in the control group(71.85%)(P<0.05).After treatment,the urine red blood cell count,24 h urine protein quantification,BUN,Scr,ALB,TG,TC,α1-MG and RBP in the treatment group were better than those in the control group(P<0.05).Compared with the control group,after treatment,the levels of IL-4 and IL-10 in the treatment group were higher,and the levels of IFN-γ,IFN-γ/IL-4 and IFN-γ/IL-10 in the treatment group were lower(P<0.05).The score of TCM symptoms after treatment and the incidence of adverse reactions in the treatment group were lower than those in the control group(P<0.05).Conclusion:Yiqi Gushe Nongsuo Pills can improve the clinical efficacy of CGN and improve the prognosis of the disease by regulating immunity,inhibiting inflammation,regulating lipid and lowering urinary protein.
