小切口汗腺切除术和微创负压抽吸术治疗腋臭患者的效果对比

Comparison of the Effect of Small Incision Sweat Gland Excision and Minimally Invasive Negative Pressure Aspiration in the Treatment of Patients with Osmidrosis

摘要 目的:对比研究小切口汗腺切除术和微创负压抽吸术治疗腋臭患者的效果。方法:根据随机数字表法将2019年9月—2021年9月来泉州市皮肤病防治院门诊部接受治疗的79例腋臭患者分为切除组(n=39)与抽吸组(n=40)。切除组患者接受小切口汗腺切除术,抽吸组患者接受微创负压抽吸术。对比两组治疗效果、伤口愈合时间、日常活动影响、复发情况及术后并发症发生情况。结果:抽吸组总有效率低于切除组(P<0.05);抽吸组伤口愈合时间短于切除组,多汗症疾病严重程度量表(hyperhidrosis disease severity scale,HDSS)评分明显高于切除组(P<0.05);抽吸组总复发率高于切除组,但两组比较,差异无统计学意义(P>0.05);抽吸组术后并发症总发生率明显低于切除组(P<0.05)。结论:微创负压抽吸术治疗效果较小切除汗腺切除术差,且后期复发率较高,但微创负压抽吸术较小切口汗腺切除术可显著改善伤口愈合情况,降低术后并发症的发生,具有较高的安全性。 Objective:To compare and study the effect of small incision sweat gland excision and minimally invasive negative pressure aspiration in the treatment of patients with osmidrosis.Method:According to random number table method,79 patients with osmidrosis who came to Quanzhou Dermatosis Prevention and Treatment Hospital from September 2019 to September 2021 were divided into excision group(n=39)and aspiration group(n=40).Patients in the excision group received small incision sweat gland excision,and patients in the aspiration group received minimally invasive negative pressure aspiration.The treatment effect,wound healing time,influence of daily activities,recurrence situation and postoperative complications were compared between the two groups.Result:The total effective rate of aspiration group was lower than that of excision group(P<0.05).The wound healing time of aspiration group was shorter than that of excision group,and the score of hyperhidrosis disease severity scale(HDSS)was significantly higher than that of excision group(P<0.05).The total recurrence rate of aspiration group was higher than that of excision group,but there was no statistical significance between the two groups(P>0.05).The total incidence of postoperative complications in aspiration group was significantly lower than that in excision group(P<0.05).Conclusion:Minimally invasive negative pressure aspiration is less effective than small excision sweat gland excision,and the later recurrence rate is higher.However,compared with small incision sweat gland excision,minimally invasive negative pressure aspiration can improve wound healing,reduce postoperative complications,and have a high safety.
