方朝晖运用小柴胡汤治疗内分泌疾病跟诊心得

小柴胡汤是临床常用方、经典方,方朝晖教授在多种内分泌疾病中运用颇多,效果显著。文章列举了方教授运用小柴胡汤加减化裁治疗甲状腺结节、糖尿病合并脂肪肝、黄褐斑验案三则,探讨方教授“抓病机”判断主证思维指导下的应用。在三则病案的治疗过程中,虽然疾病表现各异,但方教授从患者“肝气郁滞”表现切入,结合患者兼见症状,审证求因,或夹痰、或夹湿、或夹瘀,运用小柴胡汤进行治疗,并收获良好成效,达到“上焦得通,津液得下,胃气因和”的临床疗效。

Xiaochaihu Decoction is a commonly used and classic clinical prescription.Professor FANG Zhaohui has used it a lot in various endocrine diseases,and the effect is remarkable.The article lists three cases of Professor FANG’s use of Xiaochaihu Decoction to treat thyroid nodules,diabetes with fatty liver,and chloasma,and discusses the application under the guidance of Professor FANG’s thinking of“grasping the pathogenesis”to judge the main syndrome.During the treatment process of the three cases,although the disease manifestations were different,Professor FANG started from the manifestation of the patient’s“Stagnation of Liver Qi”,combined with the symptoms of the patient,examined the syndrome and sought the cause,or phlegm,dampness,or stasis,using Xiaochaihu Decoction for treatment,and achieved good results,achieving the clinical curative effect of“the upper energizer is unblocked,the body fluid is lowered,and the stomach Qi is harmonized”.
