基于中药复方治疗心肝阴虚证甲状腺功能亢进症用药规律研究

Study on Medication Regularity in the Treatment of Hyperthyroidism with Heart-liver Yin Deficiency Syndrome based on Traditional Chinese Medicine Compound

目的:基于数据挖掘技术探讨中药复方治疗心肝阴虚证甲状腺功能亢进症用药规律,进行全面分析总结,为临床诊疗提供经验参考。方法:收集整理2012年9月—2021年12月期间安徽中医药大学第一附属医院于内分泌科就诊的211例心肝阴虚证甲状腺功能亢进症患者的中药复方,通过处方信息录入科研一体化系统,进行频数分析、关联规则、聚类分析等方法观察中药的使用频次、核心药物、关联组合及聚类组方等。结果:纳入的211例患者中,女占74.9%,男占25.1%,平均年龄45岁,含中药262种,总频数为4097次。药性以温、平、寒为主,药味以甘、苦、辛为多,归经以肝经、肺经、心经及脾经为主。常用药物中茯苓、白芍、生地黄、柴胡使用频率>50%,二药关联中设定置信度>90%,支持度>80%的代表性药对关联有9个,三药关联中设定置信度>90%,支持度>70%的代表性药组有25个,聚类分析得出高频药物聚类共得到7类。结论:总结中药复方治疗心肝阴虚证甲状腺功能亢进症用药规律,重视健脾宁心,疏肝滋阴的同时也顾护脾胃生化之源,兼以益气、散结、固表、活血的中药治疗。

Objective:Based on data mining technology,to explore the rule of Chinese medicine compound prescription in the treatment of hyperthyroidism with heart-liver Yin deficiency syndrome,conduct a comprehensive analysis and summary,and provide experience reference for clinical diagnosis and treatment.Methods:From September 2012 to December 2021,the traditional Chinese medicine compound prescriptions of 211 patients with hyperthyroidism with heart-liver Yin deficiency who were treated in the Department of Endocrinology in the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine were collected and entered into the scientific research integration system through prescription information.,carry out frequency analysis,association rules,cluster analysis and other methods to observe the frequency of use of traditional Chinese medicine,core drugs,association combinations and cluster prescriptions,etc.Results:Among the 211 included patients,women accounted for 74.9%,men accounted for 25.1%,the average age was 45 years old,262 kinds of traditional Chinese medicine were included,and the total frequency was 4097 times.The properties of the medicine are mainly warm,calm and cold,and the flavors of the medicine are mainly sweet,bitter and pungent,and the main meridians are liver,lung,heart and spleen.Among the commonly used drugs,Poria cocos,Radix Paeoniae Alba,Rehmannia glutinosa,and Bupleurum are used more than 50%of the time.In the two-drug association,there are 9 representative drug-pair associations with confidence>90%and support>80%.There are 25 representative drug groups with a confidence level of>90%and a support level of>70%.Cluster analysis shows that there are 7 categories of high-frequency drug clusters.Conclusion:Summarize the medication rules of traditional Chinese medicine compound in the treatment of hyperthyroidism with heart-liver Yin deficiency syndrome,pay attention to invigorating the spleen and calming the heart,soothe the liver and nourish Yin,and at the same time protect the source of spleen and stomach biochemistry.Chinese medicine treatment.
