参附对大鼠心肺复苏术后肺损伤的保护作用及其机制的探究

摘要 目的探讨参附注射液对大鼠心肺复苏术后肺损伤的保护作用及其潜在机制。方法清洁级雄性SD大鼠36只,随机分4组:假手术组(Sham组),Sham+参附注射液治疗组(Sham+SF组),心肺复苏组(CPR组),CPR+参附注射液治疗组(CPR+SF组)。以窒息法诱导心脏停搏,6 min后开始复苏,复苏成功后分组干预,24小时后取血及肺组织。对肺组织进行病理学分析,测定湿/干重比值,MDA含量及SOD活性,检测血清炎症因子含量,Western Blot检测肺组织相关蛋白表达水平。结果与Sham组相比,CPR组大鼠肺组织病理损伤加重,湿/干重比值增大,MDA含量增加,SOD活性降低,血清炎症因子含量升高,肺组织Bcl-2表达量降低,Bax及NF-κB-p65表达水平升高,差异有统计学意义(P均<0.05);而参附注射液治疗显著改善了肺组织病理损伤程度,降低了湿/干重比值、MDA含量,增强了SOD活性,下调了血清炎症因子浓度,促进了Bcl-2的表达,并抑制了Bax及NF-κB-P65表达。结论参附注射液治疗可减轻CPR术后肺损伤,其机制可能与抑制炎症反应及细胞凋亡有关。 Objective To investigate the protective effect of Shenfu injection on lung injury after cardiopulmonary resuscitation in rats and the potential mechanism.Methods 36 male SD rats of specific pathogen free were randomly divided into 4 groups:the sham operation group(the sham group),the sham+Shenfu injection treatment group(the sham+SF group),the cardiopulmonary resuscitation group(the CPR group)and the CPR+Shenfu injection treatment group(the CPR+SF group).The rats were induced to cardiac arrest(CA)by asphyxiation method,and the resuscitation was started 6 minutes later.If resuscitation was successful,the rats were accepted different intervention in accordance with the particular group.Lung tissue was collected 24 hours after successful resuscitation,as well as blood samples,through cardiac puncture.The lung tissue was histopathologically analyzed,the wet/dry weight ratio,MDA content and SOD activity of lung tissues were measured,and the concentration of inflammatory factors in serum were detected.Western Blot was used to determine the expression of related proteins in lung tissues.Results Compared with the sham group,the pathological injury of lung tissue in CPR group was aggravated,wet/dry weight ratio was raised,MDA content was elevated,SOD activity was decreased,the concentration of serum inflammatory factors were increased,the expression level of Bcl-2 was down-regulated,but the content of Bax and NF-κB P65 were up-regulated,and the differences were all statistically significant(P<0.05).However,treatment with SF injection significantly improved the pathological injury of lung tissue,decreased wet/dry weight ratio,MDA content,increased SOD activity,reduced the content of serum inflammatory factors,promoted the expression of Bcl-2,and inhibited the expression of Bax and NF-κB P65 in lung.Conclusion Adjuvant therapy with Shenfu injection could alleviate lung injury after CPR,and its potential mechanism may be related to inhibiting inflammatory response and apoptosis.
