中老年人群中内脏脂肪体积与大脑灰质体积的相关性及其性别差异

Association between Visceral Adipose Tissue Volume and Gray Matter Volume and its Gender Difference in Middle-aged and Elderly Population

摘要 目的:探索中老年人群中内脏脂肪(visceral adiposetissue,VAT)体积与大脑灰质体积之间的相关性及其性别差异。方法:使用英国生物样本库(UK Biobank)资源进行分析。VAT与灰质体积数据下载自生物样本库中利用全身MRI和头部MRI图像分析处理后的现有数据。采用多元线性回归模型分析VAT体积与灰质体积之间的相关性;协变量包括年龄、性别、吸烟、饮酒、身体质量指数(body massindex,BMI)、腰臀比、运动量、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、甘油三酯、高血压病史、糖尿病病史及心脏病病史等。结果:最终23310人纳入分析,其中男性11052人,女性12258人。在全部人群中,多元线性回归模型结果显示,校正BMI、腰臀比等体脂因素后,VAT依然与灰质体积独立负相关(β=-2.04,95%CI-3.24~-0.84,P=0.001)。男性和女性参与者在年龄、吸烟饮酒史、BMI、腰臀比等基础资料均有显著性差异(P<0.001),相较于女性,男性有更低的大脑灰质体积[(810.82±45.90)cm^(3)vs.(777.88±44.64)cm^(3),P<0.001]及更高的内脏脂肪体积[(2.62±1.50)L vs.(4.90±2.31)L,P<0.001]。亚组分析结果显示,仅在男性人群中,VAT与灰质体积独立相关(β=-2.95,95%CI-4.46~-1.44,P<0.001)。结论:VAT与中老年人的大脑灰质体积呈负相关,而且这种相关性在男性人群中更为明显。 Objective:To explore the association between visceral adipose tissue(VAT)volume and gray matter volume in middle-aged and elderly population.Methods:This study utilized the UK Biobank resource.Multivariate linear regression models were used to analyze the correlation between VAT and gray matter volume,with covariates including age,sex,smoking,alcohol consumption,body mass index(BMI),waist-to-hip ratio,physical activity,HDL cholesterol,LDL,cholesterol,triglycerides,history of hypertension,diabetes,and cardiovascular disease.Results:A total of 23310 participants,including 11052 males and 12258 females,were included in the final analysis.The multivariate linear regression model showed that VAT was independently correlated with gray matter volume even after adjusting for body fat factors such as BMI and waist-to-hip ratio(β=-2.04,95%CI-3.24~-0.84,P=0.001).There were statistical differences in baseline characteristics,such as age,smoking and alcohol history,BMI,and waist-to-hip ratio,between male and female participants(P<0.001),with males having smaller gray matter volume[(810.82±45.90)cm^(3)vs.(777.88±44.64)cm^(3),P<0.001]and higher VAT volume than females[(2.62±1.50)L vs.(4.90±2.31)L,P<0.001].Subgroup analysis showed that only in males,VAT was independently correlated with gray matter volume(β=-2.95,95%CI-4.46~-1.44,P<0.001).Conclusion:VAT is negatively correlated with gray matter volume in middle-aged and elderly population,and this correlation is more apparent in males.
